Student Waiver & Release of Liability

Thank you for choosing Stewart Dance School! We are excited to have you as part of our dance family. Below are the terms and conditions for enrollment, payment, and participation at our school. Please read them carefully before completing your registration:
Registration Fees:
The registration fee is $60 per class. This fee includes a $50 costume deposit, which will be applied toward the cost of the costume for the June production.
Please note: Registration fees, costume deposits, and payments are non-refundable and non-transferable for dancers who withdraw before the June production.
Payment Plan & Billing:
I agree to keep a valid credit card on file in my Studio Pro account and enroll in the automatic/recurring payment plan.
Tuition is posted to accounts on the 1st of each month. Automatic payments will be charged to the card on file on the 9th of each month.
A $20 late fee will be applied to overdue accounts after the 9th of each month.
Missed Classes & Withdrawals:
Stewart Dance School does not provide credits or refunds for classes missed due to holidays, vacations, illness, weather, or other reasons.
If you wish to withdraw from classes, a 30-day notice is required.
Legal & Collection Terms:
In the event of default of payment, I agree to pay any Legal Interest on the balance due, along with any collection costs, reasonable attorney fees, and court costs incurred to collect this account.
Injury & Medical Authorization:
I understand that dance and related activities involve a risk of injury. I agree not to hold Stewart Dance School (its owners, staff, and related parties) responsible for injuries or damages incurred by any of my family members while participating or visiting the facility.
In the event of injury or illness, I authorize medical care for my child and accept responsibility for any medical expenses.
Drop-Off and Pick-Up Policy:
Students may not be dropped off more than fifteen (15) minutes prior to their scheduled class time and must be picked up no later than fifteen (15) minutes after class concludes. Dancers who remain at the studio between classes are expected to behave respectfully toward others, staff, and studio property at all times. Failure to comply with these policies, without prior communication and approval from studio staff, will result in a childcare/supervision fee of $1.00 per minute for any time exceeding the allotted drop-off or pick-up window.
Media Release:
The school is granted permission to take photographs and videos of my child and use them in brochures, websites, social media, posters, advertisements, and other promotional materials the school creates.
Permission is also granted for the school to copyright such photographs in its name.
Dismissal Policy:
Lee Anne Stewart Dance School reserves the right to dismiss any student at its discretion.
Stewart Dance School respects the privacy of our students and families. Personal information collected during registration, including contact information, medical information, and payment details, is used solely for studio operations, communication, scheduling, billing, and student safety. We do not sell or share personal information with outside parties except as necessary to provide services or comply with legal obligations.
By completing your registration, you acknowledge that you have had sufficient opportunity to read this entire document, you understand its contents, and you agree to be bound by its terms.
