Liability Waiver

I, Parent/guardian and student, recognize the potential for risks and hazards when participating in dance class activities. I freely accept and fully assume responsibility for all such risks, dangers, hazards, and the possibility of personal injury, property damage, or loss resulting from all activities my child participates in.
I voluntarily agree to release Hearts in Motion LLC, its employees, and volunteers from any and all liability for any loss, damage, injury or expense that I or my child(ren) may suffer as a result of participation in dance and activity classes.
If I cannot be contacted, I authorize Hearts in Motion LLC to seek medical services in case of injury or serious illness. I further agree to accept financial responsibility in excess of the benefits followed by my health plan.
