ENROLLMENT WAIVER

I understand and agree that Prodigy 17 Dance Company and the instructors are not liable for personal injuries or loss of property. Parent understands and expressly assumes all risks involved in connection with dance instruction, competition, and training at Prodigy 17 Dance Company. This includes but is not limited to the risk of bodily injury which may occur as a result of contact with other students and/or equipment. Each student may decline to participate in an activity which may be harmful and is responsible to inform the instructor of any physical limitations which may prevent full participation in the class.
I agree to be responsible for reading studio correspondence and respecting deadlines, if applicable.

I further give Prodigy 17 Dance Company permission to use my child's likeness and name(s) for publication, advertising, or publicity.
I am aware of the payment schedule set forth by Prodigy 17 Dance Company and by signing agree to be responsible for said scheduled payments for the full 9 months of instruction.
I understand all tuition and fees paid are NON-REFUNDABLE and NON-TRANSFERABLE and further understand and agree I must give 30 day written notice to drop from classes in order to not continue to be charged for the following months.
I understand there is a drop fee associated anytime a class is dropped from the enrollment.
I understand there is a change class fee associated anytime a class is changed to another class after enrollment is accepted.
I have read the rules and regulations, dress code and calendar for Prodigy 17 Dance Company and understand and agree that enrollment indicates acceptance and compliance of the program.
