Confidentiality Agreement
Dream Detroit Skating Club and Academy
This agreement is made between Dream Detroit Skating Club and Academy (“DDSCA”), the undersigned Parent/Guardian and Skater (“Participants”), and is effective as of [Date].
1. Purpose
To create a safe, supportive, and respectful environment for all skaters, families, and staff by keeping sensitive and private information confidential.
2. What is Confidential Information?
Confidential Information includes, but is not limited to:
• Personal information about skaters, families, or staff.
• Medical, academic, or performance-related information shared with the club.
• Training methods, choreography, lesson plans, and other program materials.
• Discussions, observations, or communications within classes, practices, or team activities.
3. Parent/Skater Agreement
By signing this agreement, the Parent/Guardian and Skater agree to:
• Respect the privacy of all skaters, families, and staff.
• Not share or post confidential information (online, in conversations, or otherwise) without permission.
• Use any information provided by DDSCA only for the purpose of participation in its programs.
• Ask permission before sharing photos, videos, or recordings of other skaters or coaches.
4. Exceptions
This agreement does not apply to information that is:
• Already public,
• Shared with written consent, or
• Required to be disclosed by law.
5. Duration
This confidentiality commitment remains in effect during and after participation in DDSCA programs.
