To finalise your dancer's enrolment, parents / guardians are required to complete the forms below:

DARE 2 DANCE – TERMS OF ENROLMENT & PARTICIPATION WAIVER
By enrolling in Dare 2 Dance (D2D), I acknowledge and agree to the following Terms of Enrolment, Participation Waiver, Safety Requirements, and Studio Policies.
1. Enrolment, Fees & Payments
• I understand that there is an annual Registration Fee payable upon enrolment. Enrolment cannot be finalised until payment is received.
• I understand that all fees are payable termly in advance, in accordance with SSC and CAC term dates. As a courtesy, D2D allows a two-week grace period for term fees to be finalised before late fees apply.
• I understand that payment may be made via the D2D App, cash, bank transfer, or the EFTPOS facilities at the studio. Merchant fees apply to EFTPOS and App payments.
• I understand that a late payment fee of $50.00 will apply for fees outstanding after Week Two (2) of any given term. Fees unpaid by the end of Week Six (6) will incur an additional $100.00 late fee.
• I understand that if fees remain unpaid for a full term, my child may not be permitted to attend classes the following term until all fees (including late fees) are paid in full.
• I understand that there are no refunds or credits for missed classes; however, students are welcome to arrange a make-up class in a comparable class, if available. Parents must email D2D to arrange this.
• I understand that written notice must be given prior to the commencement of a new term if I wish to withdraw from classes; otherwise, I am liable for the full term’s fees.
• I understand that all students are invited to participate in our end-of-year concert, Showtime. I acknowledge that involvement in the concert requires the purchase of costumes, accessories, hair pieces, and tickets, and that these additional fees must be paid in full prior to participation.
2. Attendance & Communication
• I understand that D2D staff must be notified via the D2D App if my child is absent from class.
• I understand that D2D communicates important updates via email and the D2D App, and it is my responsibility to stay informed.
• I understand that all students participate in the annual Showtime Dance Concert, and that this involves a commitment to both the matinee and evening shows.
3. Uniform & Presentation Expectations
• I understand that all Kindy–Year 6 students must wear the designated Dare 2 Dance uniform in accordance with the Uniform Requirements Sheet. Students in Years 7–12 are permitted to wear dance-appropriate activewear that supports safe participation in all class activities.
• Students are required to attend class neatly presented, with hair securely always tied back and off the face, to maintain safety, comfort, and professionalism.
4. Choreography & Studio Property
• I understand that all choreography, music mixes, and dance sequences remain the intellectual property of D2D and may not be used, shared, taught, or published without written permission.
5. Studio Rights
• I understand that Dare 2 Dance reserves the right to discontinue enrolment if behavioural, safety, or payment conditions are not met.
6. Participation Waiver & Release
General Acknowledgement & Assumption of Risk
• I understand that dance, fitness, Pilates, stretching, and conditioning involve inherent physical risks, including the risk of injury (Aerial Participation Waiver & Release on a separate form).
• I voluntarily assume all risks associated with participation in D2D classes, workshops, rehearsals, performances, or events.
• I accept full responsibility for my own (or my child’s) health, wellbeing, and safety during all D2D activities.
Release of Liability
• I release and indemnify Dare 2 Dance, its directors, employees, teachers, staff, and representatives from all claims, demands, actions, damages, or liabilities arising from participation in any D2D activity, except where directly caused by D2D’s proven negligence.
Medical Consent
• I understand that all Dare 2 Dance (D2D) staff act in accordance with reasonable duty of care standards and will respond appropriately if illness, injury, or a medical incident occurs during classes, rehearsals, performances, workshops, or events.
• In the event of injury or illness, I authorise D2D staff to administer basic first aid (such as ice, band-aids, rest, or immobilisation) if deemed necessary.
• If an incident requires further attention, I authorise D2D staff to seek emergency medical help on my (or my child’s) behalf. This may include: Calling 000 for ambulance attendance, Requesting paramedic assessment and / or Transport to a medical facility if required.
• I understand that D2D staff will make every reasonable effort to contact me (or the listed parent/guardian) as soon as practicable in the event of an injury or emergency. If I am not immediately reachable, I acknowledge that staff may proceed with medical decisions in the best interest of the child until contact is made.
• I accept full financial responsibility for any emergency or medical treatment required, including ambulance fees, transportation, or follow-up care.
• I understand that it is my responsibility to ensure D2D has up-to-date medical information, including allergies, asthma plans, injuries, or other conditions relevant to my (or my child’s) safe participation.
• I acknowledge that D2D staff are not medical professionals and that any assistance or decisions made during an incident are based on reasonable judgement, standard first aid training, and immediate necessity.
Code of Conduct
• I understand that it is my responsibility to familiarise myself with the Dare 2 Dance Code of Conduct, which is accessible via our website at www.dare2dance.com.au.
7. Media Consent
• I understand that social media consent is handled through a separate form and that I must opt in by completing that form.
8. Additional Notes for Children’s Enrolment
(For parents/guardians signing on behalf of a minor)
• I acknowledge that children in Kindy–Year 2 may require assistance getting changed for class or performances.
• I understand and agree that:
- My child will be changed in an enclosed, and fully supervised area designated for Kindy–Year 2 students, where children change together in a group setting and no external adults or unauthorised persons are permitted access.
- Dare 2 Dance staff will be present to supervise and provide appropriate, minimal assistance where required;
- D2D upholds strict child-safe standards at all times.
I acknowledge that I am responsible for ensuring my child meets all participation requirements, uniform guidelines, rehearsal expectations, and payment obligations.
9. Agreement
By enrolling at Dare 2 Dance, I acknowledge that:
