2024 EDT Conservatory Contract

VOLUNTEER COMMITMENT (Both parents/guardians must initial)

Parent/Guardian 1:

_______ I am committing to serve a minimum of 6 hours on the following committees (circle at least 1): 
__art/set design ___costume   ___crew ___decorating    ___event planning  ___ props   ___usher
and will work as backstage crew or dresser, or ushering at least one performance.

-OR-
_______ I cannot serve on a committee, but enclosed is my tax-deductible donation of $120.00 to Edinburg Dance Theatre

Parent/Guardian 2:

TICKET SALES COMMITMENT (one parent/guardian per family must initial)*

I will purchase/sell the minimum required tickets for each performance: 
______ Peter & the Wolf- total 8x3=24 tickets @ $12 each. Total $288 due Oct. 1. -OR- _______ $250 donation
______ Nutcracker- 7x4=28 tkts @$15 ea.=$420 -OR- _______ $400 donation
______ Irish- Total 8x2=16 tickets @ $12 each. $192 due February 2. -OR- _______ $160 donation

-OR-

_______ Enclosed is my tax-deductible donation of $825 to help with production costs. I understand that I am not obligated to assist with ticket sales, and I may receive 2 complimentary tickets for each performance.
-OR-

_______ Enclosed is my tax-deductible donation of $275. I will make 2 additional payments of $275 On Oct. 1 and Nov. 1.  I understand that I am not obligated to assist with ticket sales, and I may receive 2 complimentary tickets for each production.

FUNDRAISING COMMITMENT (per family)

I will participate in both EDT fundraisers: 

______popcorn sale (100 bags @ $2 each=$200 due Sept. 26)   OR     ______Donation of $100

AND

_____ Plate Sale (10 tickets @ $10 each=$100 due Oct 25).   OR     ______Donation of  $ 75
 
PHOTOGRAPHIC RELEASE:
We understand and agree that for promotional, publicity, or commercial advertising purposes, Edinburg Dance Theatre shall have the right to use a photograph, video recording, voice, or other likenesses of the dancer.

Participation in this organization is contingent upon the rules set forth in this contract.  If at any time during the performance season we fail to fulfill our obligations, we understand that participation will be terminated.  

I have read and understand the requirements of participation in Edinburg Dance Theatre and agree to abide by them.
