ENCORE DANCE STUDIO
Assumption of Risk, Release of Liability, Media Release & Payment Agreement
ASSUMPTION OF RISK
I understand that participation in dance classes, rehearsals, performances, competitions, workshops, travel, and other studio-related activities involves physical activity and carries an inherent risk of injury.
Such risks may include, but are not limited to, muscle strains, sprains, fractures, concussions, illness, property damage, serious injury, or, in rare circumstances, death.
I acknowledge that it is my responsibility to ensure that my child is physically capable of participating in dance activities. I understand that Encore Dance Studio is not responsible for determining my child's fitness to participate.
By enrolling my child in Encore Dance Studio programs and activities, I voluntarily assume all risks associated with participation.

RELEASE OF LIABILITY
In consideration of my child's participation in activities offered by Encore Dance Studio, I hereby release, waive, and forever discharge Encore Dance Studio, its owners, directors, employees, contractors, teachers, assistants, volunteers, agents, and representatives from any and all claims, demands, causes of action, damages, losses, costs, expenses, or liabilities arising from or related to participation in any studio activity.
This release includes, but is not limited to, claims arising from personal injury, illness, medical treatment, property damage, or other losses that may occur during participation in classes, rehearsals, performances, competitions, events, or travel related to those activities.
I authorize Encore Dance Studio and its representatives to seek emergency medical treatment for my child if I cannot be reached. Any costs associated with such treatment remain my responsibility.
I further agree to indemnify and hold harmless Encore Dance Studio from any claims, damages, losses, liabilities, or expenses arising from my own negligence or willful misconduct.

HEALTH & SAFETY RESPONSIBILITIES
I certify that my child is physically fit, in good health, and able to safely participate in all activities offered by Encore Dance Studio.
I understand that it is my responsibility to disclose any medical conditions, injuries, allergies, physical limitations, behavioural concerns, or other health-related information that may impact my child's participation.
All health-related information must be submitted directly to the studio by email. Verbal conversations with teachers, assistants, volunteers, or other studio representatives do not constitute official notification.
I agree to promptly notify the studio of any changes to my child's health status throughout the dance season.
I agree not to send my child to classes, rehearsals, performances, competitions, or other studio activities when they are experiencing symptoms of illness that may place others at risk.
Encore Dance Studio reserves the right to refuse participation if it believes a dancer's attendance may pose a health or safety concern.
I further acknowledge that participation in studio activities may expose my child to communicable illnesses, including but not limited to colds, influenza, COVID-19, and other viruses. I voluntarily assume any risks associated with such exposure.

PHOTO & VIDEO RELEASE
I understand that photographs and videos may be taken during classes, rehearsals, performances, competitions, events, and other studio-related activities.
I grant permission for Encore Dance Studio to use photographs and/or video recordings of my child for promotional, educational, advertising, and marketing purposes, including but not limited to:
* Social media platforms
* Studio websites
* Email communications
* Printed materials
* Advertisements
* Posters
* Studio publications
I understand that no compensation will be provided for the use of such photographs or recordings.

TUITION, FEES & PAYMENT POLICIES
I understand that registration constitutes enrollment for the full dance season as outlined in the current studio policies.
Competitive Programs
After **August 1**, all tuition, rehearsal fees, competition entry fees, and costume fees become fully due and payable.
No refunds, credits, transfers, or cancellations will be provided for tuition, rehearsal fees, entry fees, or costume fees after August 1.
Recreational Programs
After **October 14**, all tuition and costume fees become fully due and payable.
No refunds, credits, transfers, or cancellations will be provided for tuition or costume fees after October 14.
Outstanding Balances
A late fee of **3% per month** will be applied to any account balance remaining unpaid as of June 1.
A **$20 fee** will be charged for any returned or dishonoured payment.
I acknowledge that I have read, understood, and agreed to the current Encore Dance Studio Policies, which are available on the studio website and distributed electronically.

ELECTRONIC SIGNATURE AGREEMENT
By selecting "I Accept" or otherwise submitting this agreement electronically, I acknowledge that I am signing this agreement electronically and that my electronic signature has the same legal effect as a handwritten signature.
I confirm that I have carefully read this entire agreement, understand its contents, and voluntarily agree to be bound by its terms and conditions.
I understand that this agreement is binding upon myself, my child, and our respective heirs, executors, administrators, and legal representatives.
