2026-2027 WAIVER AND RELEASE FROM LIABILITY FORM

By acknowledging this waiver you are stating you read our 2026-2027 Waiver and Release from Liability Form. You agree to the terms, and are permitting your child to participate in dance instruction at Palmyra Academy of Dance Inc.

I acknowledge that I understand the nature of the activities my child will be participating in and that my child is in the proper physical condition and capable of participating in the related activities, understanding that Palmyra Academy of Dance Inc. is not in any way responsible for making such a determination.

In consideration of my child’s enrollment in any dance instruction program, I understand and agree on behalf of myself and my child, to release, hold harmless, and discharge Palmyra Academy of Dance Inc. from all claims, cost, liabilities, expenses, or judgements, including attorneys’ fees and court cost for any occurrences in connection with any dance instruction.

I assume all risk to my child in connection with any instruction and further release Palmyra Academy of Dance Inc. and its owners and employees from liability for any injury sustained by my child while he or she is enrolled in any dance instruction program, including all risk reasonably connected with such activity whether foreseen or unforeseen.

I understand that Palmyra Academy of Dance Inc. is not responsible for my child or other children under my supervision who are left unsupervised in the common areas and areas surrounding the dance studio and that Palmyra Academy of Dance Inc. will only be supervising my child when he or she is participating in scheduled dance activities, programs or instruction.

I understand that Palmyra Academy of Dance Inc. is not responsible for personal property that is lost, damaged or stolen while I or my child is at or on Palmyra Academy of Dance Inc. property.

I acknowledge and agree that it is my responsibility to maintain my own accident and health insurance coverage that provides adequate coverage for myself and my child participating in Palmyra Academy of Dance Inc. activities and that Palmyra Academy of Dance Inc. does not provide accident or health insurance for those participating in it’s instruction, activities or programs.

I authorize and agree that Palmyra Academy of Dance Inc. may take and use photographs, videos or likenesses of myself or my child as needed for its record-keeping, advertising and/or public relations projects and that I have no rights to the same and will not be compensated for the same.

My registration is proof of my intention to execute a complete and unconditional waiver and release of all liability pursuant to the terms herein, and agreement as to all terms and conditions contained above. I am of lawful age and competent to sign this affirmation.

2026-2027 STUDIO POLICIES ______________________________________________________________________________________________________________________________
REGISTRATION
A non-refundable registration fee of $45 and one-month tuition to be applied to JUNE 2027 tuition is due at the time of registration (September tuition is due the week of September 14)
Checks can be made payable to Palmyra Academy of Dance.
TUITION
Tuition is due before the 5th of each month. After the 15th, a $10 late fee will be applied. Students cannot take classes if their tuition is more than 60 days late. All tuition must be paid in full for students to participate in the end-of-year show.
A $25 fee will be charged for a returned check marked insufficient funds.
Refunds will not be given for missed classes. Make-up classes are available through November only and must be coordinated with Studio staff by email or phone.
We understand the importance of convenience in managing your child's dance education. That's why we offer multiple payment options. Tuition can be paid online in your Parent Portal. For those who prefer traditional methods, Cash and checks can be placed in mailbox #1 at the studio entrance, or checks can be mailed to 1 West Main St., Palmyra, PA 17078.
PLACEMENT POLICY: Online Registration will be reviewed and confirmed. Students are placed in classes that our instructors deem age and level-appropriate. We recognize the balance between skill and age and may change a student’s level. We will reach out if a level change is necessary.
ATTENDANCE POLICY: We expect students to arrive 5 minutes early for class and dress according to that specific class’s dress code. Students with frequent absences fall behind, so regular class attendance is crucial as the curriculum will build on the previous week’s instruction.
SPECIAL STUDENTS, SPECIAL NEEDS: At Palmyra Academy of Dance, we are committed to providing a supportive environment for all students. We require written documentation for any student with special needs prior to registration. We will discuss how and if your child’s needs can be met. The opportunity to express oneself through dance is very beneficial, and we will do our best to share that with all.
SHOW: Show participation is not required. Our show dates are Friday, June 4 and Saturday, June 5, 2027 at The Hershey Theatre. Students in all class levels will participate in both shows. Rehearsals will be held at the theater from May 31 through June 3. Rehearsal attendance is mandatory for participation in the show. Your child’s rehearsal schedule will be available in March 2027. Please let us know as soon as possible if there are any conflicts. Costumes and tickets are required to be purchased. There is a show fee of $115 per dancer or $150 per family to participate in the show. This fee can be paid directly or fundraised. A non-refundable costume deposit is due in November.
STUDENT WITHDRAWALS: If you need to withdraw your child from our program, we require a written drop notification by email. Please note that parents are responsible for paying all remaining costume balances for any withdrawal after November 12, 2026.
DRESS CODE: Our instructors need to be able to see the dancers’ bodies at work. Therefore, students will be required to wear specific attire and shoes for each class. Hair should be worn up and out of the dancer’s face. Ballet students must have their hair in a bun. All required items can be purchased through the studio. Hip Hop students will need to buy sneakers that coordinate with their costumes. The studio will send specific information.
PARENT TEACHER COMMUNICATION: Please do not use drop off or pick up to communicate questions to teachers. The best form of communication is via email. To maintain a positive and productive relationship, we ask that all communication be respectful, professional, and focused on the needs of the child. Open and honest dialogue is always welcome, but it must be conducted in the spirit of mutual respect.
