By enrolling at Studio 4 Dance, you understand and acknowledge that dance and physical activity involve inherent risks, including the risk of injury, which may include but are not limited to sprains, strains, fractures, and other physical injuries. Parents or guardians voluntarily assume all risks associated with their child’s participation in dance classes, rehearsals, performances and related activities at Studio 4 Dance.
In consideration of being allowed to participate in classes and activities at Studio 4 Dance, the student (or their parent/guardian if under 18) hereby releases and holds harmless Studio 4 Dance, its owners, employees, and teachers from any and all liability, claims, demands, or causes of action for any injuries, damages, or losses that may arise from participation in any classes, events, or activities, except in cases of gross negligence or wilful misconduct.
By enrolling you confirm that your child is physically capable of participating in dance activities. Students (or parents or guardians, if the student is a minor) are required to notify the studio of any medical conditions, injuries, or limitations that may affect participation. In the event of an emergency, you authorise Studio 4 Dance to seek medical treatment if necessary.
