Dynamic Dance Academy / Dynamic Angels Health & Safety Dance Waiver

Thank you for being a valued member of the Dynamic Angels Dance Team and/or a student of Dynamic Dance Academy classes this dance season/session. We are committed to providing a safe and healthy environment for all our dancers, families, and staff. While we understand that risks associated with COVID-19 and in-person gatherings still exist, we are taking every reasonable measure to promote safety.

Please carefully read the following rules and policies put in place to help protect our community during these ongoing times. Your review and signature indicate your understanding and agreement.

Health & Safety Acknowledgment

I understand that I cannot hold the coaches, parent committee members, or other members of the dance team liable for my own health or the health of my child as I give consent for my dancer to attend practices.

I acknowledge that Dynamic Dance Academy has implemented safety protocols, which may include:

Emergency contact procedures

Temperature checks as needed

Hand sanitizing stations

Social distancing when possible

Confidential notification in the event of possible exposure

I understand that dance is a physical and contact-based activity, and while distancing will be encouraged when feasible, it may not always be possible.

I understand that, in the interest of team safety, practice may be canceled if there is a potential exposure or health risk, and will resume only when it is deemed safe.

I agree that if I or a member of my household has been exposed to COVID-19, displays symptoms, or feels unwell for any reason—even if we do not suspect COVID-19—we will stay home from practice. I will also report any symptoms or exposure to Coach Bre.

I understand that Coach Bre will maintain confidentiality and will not share individual names, but will notify the team and parents if an exposure has occurred or if a temporary pause in practice is necessary.

By signing below, I give my child permission to participate in the Dynamic Angels Dance Team and/or Dynamic Dance Academy classes for the current dance season/session. I agree to follow all current state guidelines and the safety measures implemented by the organization.
