CLASS EXIT FORM POLICIES
I, ________________________________ understand that thirty (30) days notice must be given in writing before any student can exit a class. I agree that if thirty (30) days notice is NOT received in writing by Dance Xpress, I am RESPONSIBLE for payment and will continue to be billed until written notice is received by Dance Xpress.
I understand that I am responsible for tuition payment for the next month if I exit on or after the 10th of the month.
