RULES OF CONDUCT & REGULATIONS
Starz Elite Performance Academy
1. Respect & Behavior
• Students must treat instructors, staff, teammates, and themselves with kindness and respect.
• Bullying, teasing, exclusion, or disruptive behavior is strictly prohibited.
• A positive attitude and supportive behavior are expected at all times.
2. Attendance & Punctuality
• Students should arrive on time, dressed appropriately, and ready to participate.
• Excessive absences or tardiness may affect class placement, skill progression, or participation in performances.
• Parents should notify the academy in advance when absences are necessary.
3. Proper Attire
• Students must wear approved dance or gymnastics attire suitable for safe movement.
• Hair must be neatly secured away from the face.
• Jewelry, jeans, belts, and loose clothing are not permitted for safety reasons.
• Bare feet or appropriate dance/gymnastics footwear only.
4. Safety Expectations
• Students must follow all instructions from coaches and staff at all times.
• Running in the facility, using equipment without permission, or engaging in unsafe behavior is not allowed.
• Students should report any injuries, discomfort, or concerns immediately.
5. Facility Rules
• Only water is permitted in training areas. No food, gum, or drinks other than water should be in the training areas..
• Personal belongings must be stored in designated areas.
• Students and families are expected to help maintain a clean, respectful environment.
6. Parent & Guardian Conduct
• Parents must remain respectful toward staff, students, and other families.
• Coaching from the sidelines, interrupting classes, or entering training areas without permission is not allowed.
• Concerns should be addressed privately with staff at an appropriate time.
7. Media & Photography
• Recording or photographing students during class is not permitted unless approved by staff.
• The academy may take photos or videos for promotional use with prior consent from families.
8. Commitment & Effort
• Students are expected to participate fully, listen carefully, and give their best effort.
• Consistent practice and a positive work ethic support safe skill development and progress.
9. Discipline Procedures
• Failure to follow rules may result in:
• Verbal warnings
• Parent meetings
• Temporary removal from class
• Suspension or dismissal from the program (for repeated or severe issues)
10. Agreement
By enrolling in Starz Elite Performance Academy, students and parents agree to follow all rules and regulations to ensure a safe, positive, and productive environment for everyone.

Assumption of Risk
By signing this document, you acknowledge that your minor child and/or you will be voluntarily participating in supervised, organized activities sponsored by Starz Elite KY, LLC. You understand that participation in these activities involves inherent risks, which may include, but are not limited to:
• Sprains
• Bruises
• Pulled muscles
• Broken bones
By allowing participation, you voluntarily accept these risks on behalf of your minor child and/or yourself. You further agree to release and hold harmless Starz Elite KY, LLC—including its owners, operators, teachers, fellow students, families, and employees—from any injuries, illnesses, or damages incurred while participating in Starz Elite activities. This release applies to you, your minor child, and your heirs and successors.
You also acknowledge that Starz Elite, LLC is not responsible for any personal injury or property loss to students, families, or guests that may occur in connection with participation in these activities, even if such injury or damage is caused in whole or in part by the negligence or fault of any released party.
Release of Liability
By signing this document, you acknowledge the inherent risks associated with the described activities. These activities should not be attempted without knowledgeable supervision, proper spotting, and appropriate safety equipment such as well‑matted areas.
Payment Policies
By signing this document, you acknowledge and agree to the following payment terms:
• Starz Elite offers classes by a full season price.
Once you enroll in a class, the registration fee and full season tuition must be paid in full, either up front, or through the payments specified when registering, or if other written arrangements have been agreed upon with management. Tuition payments are due as scheduled, regardless of the number of classes attended and breaks and holidays in the season.
We do not offer month to month lessons
We must have 4 weeks written notice to withdraw your student from the class during any season. Any fees those last 4 weeks must be paid, including your last payment before withdrawal.
Once your season has ended you are no longer a student until you sign up for the following season. Our seasons currently run Mid-August to Mid-May, and Mid May to Early August.
Returned checks are subject to a $35 returned check fee.
Registration fees are non‑refundable.
If an account is more than 1 month past due, the balance must be paid to continue in the class.
Credit Card / Bank Account Requirement
All accounts must have a credit card or bank account on file, unless the full balance is paid in advance as arranged with Management in writing.
Commercial Use Policies
By signing this document, you grant permission to Starz Elite KY, LLC to use your student’s and/or your audio or visual image in print or digital/electronic form—including photographs, audio recordings, videos, television, or other media—for:
• Advertising
• Promotions
• News coverage
• Commercial use
• Educational activities
• Event documentation
This permission applies to all Starz Elite events and activities.
