White Swan Academy Privacy Policy & Liability Waiver

Welcome to White Swan Academy. By registering yourself or your child for classes, camps, workshops, performances, or other activities, you acknowledge and agree to the following terms:

Participation & Assumption of Risk

I understand that dance training and related activities involve physical activity and may carry a risk of injury. I acknowledge that my child (or I) is physically capable of participating in dance instruction and related activities, and that White Swan Academy is not responsible for determining a participant’s physical fitness or medical condition.

In consideration of enrollment in any program offered by White Swan Academy, I voluntarily assume all risks connected with participation, whether foreseen or unforeseen.

Release of Liability

I agree to release, waive, hold harmless, and discharge White Swan Academy, its owners, instructors, staff, and employees from any and all claims, liabilities, costs, expenses, damages, or injuries arising from participation in studio activities, including travel to and from the studio or studio-related events.

I understand that White Swan Academy is not responsible for any lost, stolen, or damaged personal property while on studio premises or at studio events.

Supervision Policy

I understand that students are supervised only during their scheduled class or activity times. White Swan Academy is not responsible for children left unattended in waiting areas, hallways, parking lots, or surrounding premises before or after class.

Medical & Insurance Responsibility

I understand that White Swan Academy does not provide medical, accident, or health insurance coverage for participants. I acknowledge that it is my responsibility to maintain appropriate insurance coverage for myself and/or my child.

In the event of an emergency, I authorize White Swan Academy staff to seek appropriate medical care for my child if I cannot be reached immediately.

Photo & Media Release

I authorize White Swan Academy to photograph or record myself and/or my child during classes, rehearsals, performances, competitions, or studio events. I understand these images or videos may be used for studio purposes including social media, website content, advertising, recital programs, and promotional materials without compensation.

Classroom Protocol

I understand that professional dance instruction may include appropriate physical corrections and “hands-on” guidance from instructors as part of safe and effective dance training. If there are any physical, medical, emotional, or personal limitations regarding physical contact, I understand it is my responsibility to notify the instructor and studio in writing.

Privacy Policy

Personal information collected during registration, including contact information and emergency contacts, will be used solely for studio operations, communication, scheduling, billing, and studio-related announcements. White Swan Academy does not sell or share personal information with outside parties except when required by law.

Acknowledgment & Agreement

By completing registration, I acknowledge that I have carefully read and fully understand this Privacy Policy & Liability Waiver. I voluntarily agree to all terms and conditions stated above.
