Please review and accept Nebraska City Dance Studio's Liability & Risk policy.

I have received and read the Nebraska City Dance Studio Policies and agree to have my dancer(s) as well as the parent/guardians of the dancer(s) adhere to the policies. My dancer(s) will participate at his/her own risk and will not hold Nebraska City Dance Studio, it's directors, employees or agents responsible for loss, claim or injury as a result of his/her participation. In the event of an accident or injury and I am unable to be reached, I give my permission for treatment as deemed necessary by the board, staff or emergency personnel.
