CBDA TERMS & CONDITIONS

CBDA PRIVACY POLICY:
CBDA does not retain or share any personal data or banking information submitted for fee payments via email transfer, cheques or credit card information we collect from you. We will protect your personal data and will only share it with your written consent or under lawful basis circumstances. When accessing all online sites associated with CBDA, we only use cookies and similar technologies to manage cookies settings to personalize your user experience with our products and services.

CBDA REGISTRATION AGREEMENT:
In adherence with C.B. Dance Academy (CBDA) Privacy Policy, I give my written consent to CBDA to use, without liability, pre-identified personal information of myself, and/or the student named above in order for CBDA to provide me or the above student with dance / performance / costume / registration services - including by email.

Students and parents automatically grant permission to CBDA and its Directors to use, without any compensation whatsoever, their photos and videos for publicity and promotional purposes, including but not limited to ads, television, videotaping and broadcasting, social media, web or print. I also agree to pay to CBDA, upon registration, my child's non-refundable non-transferable dance fees, annual Administration Fee, and Costume Fee (for each recreational and/or competitive class). I am aware that costumes will be automatically ordered and billed for each child registered.

I recognize that a risk of injury or potential health risks may be involved in participation in the programs / activities in which I am registering my child. I hereby willingly assume full responsibility for such risk of injury or health risks however caused for myself or for the above-named person for whom I am by law responsible during and after my / their participation in the program(s). In consideration of CBDA accepting this application, I hereby remise, release and forever discharge CBDA, its directors, instructors, officers, administrators, successors and/or volunteers participating in or connected with CBDA of and from all manner of action, causes of action, claims or demands against CBDA, I never had, now have, or may hereafter have, for or by reason of any loss, damage or injury sustained by me or the above named person from any medical therapy treatment advised or performed, or of the loss of any equipment used by me or the above named person, whether caused by the negligence of the releasee or otherwise.

I have been provided access to CBDA's "Policy & Guidelines and Privacy Policy" and have read them thoroughly prior to electronically signing this form and and agree to go through these guidelines with my child so that he/she is aware prior to class participation. I have also read and voluntarily agree that no oral representations, statements or inducements apart from the foregoing written agreement have been made.

POLICY & PROCEDURES:
NO PEANUTS OR NUT PRODUCTS (including peanut butter) ALLOWED IN STUDIO.

Students are to arrive no earlier than 10 minutes before their class and are to be picked up no later than 10 minutes after their class. Younger students arriving early must be supervised. Teachers and CB Dance Academy staff are not responsible for students before classes commence or after they have finished.

Students should arrive dressed according to the dress code with all necessary hair accessories and shoes. Hair is to be worn back off the face for all classes. Ballet students with long hair are required to wear their hair in a bun. CBDA does not assume responsibility for lost, stolen or misplaced items. Please notify the studio in advance of a class absence.

Free Trial classes are offered to all students SUBJECT TO SPACE and CLASS AVAILABILITY in the chosen class.

Students are expected to act appropriately during class and not disrupt their teachers or other students. Parents will be notified if their child is misbehaving or causing a disruption. Upon a third notification, the child will be withdrawn from their class.

No child will be released on their own accord. For early dismissal, parents must make prior arrangements and come to the studio door to pick up their child. This is a matter of safety.

Missed classes will not be made up and no refunds shall be issued. Cancelled classes by the Studio (outside of Statutory Holidays) will be rescheduled accordingly.

Once registered, student class placement is considered as reserved for the full season. All fees, including costume deposits, are NON-REFUNDABLE, NON-TRANSFERABLE. In the case of a withdrawal from CB Dance Academy, the Director must be notified in writing.  Until such time, the space will be held for the student and charges invoiced to the parents or guardians accordingly. Upon withdrawal, fees will be recalculated at a per class rate, and the standard Withdrawal Fee of $100.00+HST per class, per student, will be processed to the account / credit card on file. Remaining balance will be credited to the account, and must be used within 365 days from date of withdrawal. After this time period, the credit will no longer by valid. All NSF payments submitted to CBDA will be assessed a $55.00+HST penalty.

Students and siblings must always be supervised. Parents of students who deface or damage studio property will be charged for the cleaning, replacement or repair of such items.

Costumes must be purchased to participate in the year-end recital. All students will perform in the CBDA year-end recital - unless Parent advises CBDA in writing at time of Registration.

All accounts must be up-to-date and in good standing in order that year-end recital tickets and costumes are made available.
