PARTICIPATION AGREEMENT, ASSUMPTION OF RISK, LIABILITY WAIVER, MEDICAL AUTHORIZATION, MEDIA RELEASE, PRIVACY NOTICE, AND STUDIO POLICIES & FINANCIAL AGREEMENT
As the parent and/or legal guardian of the student(s) identified in this registration, I hereby consent to my student(s)' participation in programs, classes, lessons, rehearsals, camps, clinics, performances, competitions, open gyms, special events, and related activities offered by Reign Dance & Cheer, LLC ("Reign").
I understand and acknowledge that participation in dance, cheerleading, tumbling, acrobatics, trampoline activities, stunting, pyramids, conditioning, strength training, flexibility training, and related physical activities involves inherent risks. These risks include, but are not limited to, falls, collisions, sprains, strains, fractures, concussions, permanent disability, illness, paralysis, and, in rare circumstances, death. I understand that these risks cannot be completely eliminated, even with proper instruction, supervision, equipment, and safety measures.
I knowingly and voluntarily assume all risks, both known and unknown, associated with my student(s)' participation in any Reign activity.
In consideration of my student(s) being permitted to participate in activities offered by Reign, I, on behalf of myself, my student(s), and our heirs, executors, administrators, and assigns, hereby release, waive, discharge, covenant not to sue, and forever hold harmless Reign Dance & Cheer, LLC, its owners, directors, employees, coaches, instructors, contractors, volunteers, agents, affiliates, partner organizations, landlords, property owners, and representatives from any and all claims, demands, causes of action, liabilities, damages, losses, costs, or expenses arising from or related to my student(s)' participation in any activity, including those resulting from ordinary negligence to the fullest extent permitted by law.
This release applies to participation in all activities conducted by Reign, whether occurring at the facility, at competitions, performances, camps, clinics, conventions, special events, off-site locations, or while using any equipment, parking areas, spectator areas, waiting areas, or other premises associated with Reign activities.
I certify that my student(s) are physically capable of participating in these activities and have no medical condition that would prevent safe participation except as disclosed to Reign. I agree to notify Reign of any injuries, illnesses, allergies, medical conditions, or physical limitations that may affect participation.
In the event of an illness, injury, or medical emergency, and if I cannot be reached promptly, I authorize Reign and its representatives to obtain emergency medical treatment for my student(s) as deemed reasonably necessary. I understand and agree that I am solely responsible for any medical expenses incurred as a result of such treatment.
I understand that participation may involve the risk of concussion or other head injuries. I agree that my student(s) will not return to participation following a suspected concussion or serious injury until medically cleared by an appropriate healthcare provider, when applicable.
I grant permission to Reign Dance & Cheer, LLC to photograph, video record, livestream, or otherwise capture images or recordings of my student(s) during classes, rehearsals, performances, competitions, events, and other activities. I authorize Reign to use such photographs, videos, and recordings for promotional, educational, advertising, marketing, website, social media, print, and digital purposes without compensation. Reign will not intentionally publish a student's full name without additional consent.
I acknowledge the contagious nature of infectious diseases, including but not limited to influenza, COVID-19, RSV, and other communicable illnesses. I voluntarily assume the risk that my student(s) may be exposed to or contract such illnesses through participation in activities and release Reign from any claims arising from such exposure or illness.
I agree to indemnify, defend, and hold harmless Reign Dance & Cheer, LLC and its affiliates from any claims, liabilities, damages, costs, or expenses, including attorney's fees, arising from or related to my student(s)' participation in activities or any breach of this agreement.
I understand that it is my responsibility to ensure my student(s) arrive on time, are dressed appropriately, follow facility rules, and participate in a safe and respectful manner.
This agreement shall be governed by and interpreted under the laws of the State of Alabama. Any legal action arising from this agreement shall be brought exclusively in a court of competent jurisdiction located in Baldwin County, Alabama.
If any provision of this agreement is determined to be invalid or unenforceable, the remaining provisions shall continue in full force and effect.
PRIVACY NOTICE
Reign Dance & Cheer, LLC collects personal information, including participant names, parent/guardian names, addresses, phone numbers, email addresses, emergency contact information, medical information provided by families, attendance records, photographs, videos, and payment-related information for the purposes of enrollment, communication, billing, safety, program administration, and participation in activities.
Information may be shared with trusted third-party service providers, including registration platforms, payment processors, communication platforms, competition and event organizers, and insurance providers when necessary to operate programs and services. Reign Dance & Cheer, LLC does not sell personal information to third parties.
Reasonable measures are taken to protect personal information; however, no electronic system can guarantee absolute security. By enrolling and participating in programs, parents and guardians consent to the collection, storage, and use of information as described in this Privacy Notice.
STUDIO POLICIES & FINANCIAL AGREEMENT
Enrollment & Registration
Enrollment reserves a space in a class, team, camp, clinic, private lesson, open gym, event, or program. Staffing, scheduling, facility usage, team placement, and program planning are based upon enrollment commitments.
Annual registration fees are non-refundable and non-transferable.
Tuition & Payment Policies
Tuition is due according to the payment schedule established by Reign Dance & Cheer.
Families are required to maintain a valid payment method on file.
Past-due balances may result in suspension from classes, practices, performances, competitions, events, or other activities until the account is brought current.
Returned payments, failed transactions, and declined payment methods may be subject to additional fees.
Credit card processing fees, transaction fees, and third-party payment processing fees are non-refundable.
Refund Policy
All payments made to Reign Dance & Cheer are non-refundable unless otherwise approved in writing by Reign Dance & Cheer.
No refunds or credits will be issued for:
* Registration fees
* Tuition payments
* Missed classes
* Illness or injury
* Vacations
* Schedule conflicts
* Voluntary withdrawal
* Competition fees
* Convention fees
* Choreography fees
* Music fees
* Costume fees
* Camps, clinics, workshops, intensives, private lessons, open gyms, or special events
Enrollment reserves a space that may not be filled by another participant if withdrawn.
Reign Dance & Cheer reserves the right to review extraordinary circumstances on a case-by-case basis. Any exception granted shall not establish a precedent for future requests.
Withdrawals
Parents or guardians wishing to withdraw a participant from any ongoing program must submit written notice to Reign Dance & Cheer at least thirty (30) days prior to the intended withdrawal date. This includes logging into Studio Pro and dropping your class(es).
Withdrawal requests are not considered received until written confirmation has been provided by Reign Dance & Cheer.
A 30-day notice period is required regardless of attendance during that period. Tuition and other applicable charges will continue to be assessed during the notice period and remain the responsibility of the family.
No refunds or credits will be issued for tuition, fees, or charges incurred during the required notice period.
Competition fees, convention fees, choreography fees, music fees, costume fees, and any other expenses previously committed, invoiced, ordered, registered, scheduled, or paid on behalf of the participant remain non-refundable and are the responsibility of the family.
Failure to attend classes, practices, rehearsals, events, or activities does not constitute withdrawal from the program and does not relieve the family of financial obligations. Written notice must be submitted and confirmed by Reign Dance & Cheer in order for withdrawal to be processed.
Attendance
Regular attendance is essential for student development, team progress, and performance preparation.
Missed classes do not qualify for refunds.
Make-up classes may be offered at the sole discretion of Reign Dance & Cheer when available.
Competition Dance Company & Allstar Cheer Programs
Participation in competitive programs constitutes a season-long commitment.
Competition registrations, convention registrations, choreography expenses, music expenses, coaching expenses, travel planning, team formations, costume orders, and related expenses are based upon participant commitments.
Competition, convention, choreography, costume, and related fees become non-refundable once committed, ordered, registered, scheduled, or paid on behalf of the participant.
Withdrawal from a competitive program does not eliminate financial obligations already incurred on behalf of the participant.
Costumes, Uniforms & Merchandise
Costume, uniform, practice wear, merchandise, and special-order items are non-refundable once ordered.
Families are responsible for providing accurate sizing information.
Communication
The Reign Dance & Cheer mobile app serves as the primary communication platform.
Parents and guardians are responsible for maintaining current contact information, downloading the Reign Dance & Cheer mobile app, enabling notifications, and regularly reviewing communications.
Official studio communications, including schedules, updates, deadlines, billing information, event details, weather closures, team information, and important announcements may be delivered through the app, email, text message, the parent portal, BAND, or other approved communication channels.
Failure to receive, read, review, or respond to communications does not waive studio policies, deadlines, financial obligations, participation requirements, or other responsibilities.
Parent & Participant Conduct
Reign Dance & Cheer is committed to maintaining a positive, safe, and respectful environment.
Disrespectful, abusive, threatening, harassing, or disruptive behavior toward staff, coaches, volunteers, participants, families, judges, event personnel, or guests will not be tolerated.
Reign Dance & Cheer reserves the right to suspend or dismiss any participant or family whose conduct negatively impacts the studio environment.
No refund shall be provided in the event of dismissal resulting from misconduct or policy violations.
Policy Changes
Reign Dance & Cheer reserves the right to modify policies, procedures, schedules, tuition rates, fees, and program requirements as necessary.
Updated policies may be communicated through the website, parent portal, mobile app, email, text message, or other official communication channels.
By continuing participation after notification of updated policies, families agree to comply with such revisions.
ACKNOWLEDGMENT
I acknowledge that I have carefully read and fully understand this Participation Agreement, Assumption of Risk, Liability Waiver, Medical Authorization, Media Release, Privacy Notice, and Studio Policies & Financial Agreement. I understand that I am waiving certain legal rights on behalf of myself and my student(s), accepting financial responsibilities associated with enrollment, and voluntarily agreeing to all terms contained herein.
By checking this box and completing registration, I acknowledge that I have read, understand, and agree to all provisions of this Agreement, including the Liability Waiver, Medical Authorization, Media Release, Privacy Notice, Refund Policy, Tuition Policy, and 30-Day Withdrawal Policy.
I certify that I am the parent and/or legal guardian of the student(s) listed and have the authority to enter into this agreement on their behalf.
