EVOLVE DANCE STUDIO – POLICIES & WAIVER
Please read carefully before enrollment.
PAYMENT & BILLING POLICIES
- AUTO-PAY REQUIRED: All accounts must be enrolled in automatic monthly payments using a credit/debit card or checking account.
- MONTHLY PAYMENT SCHEDULE: Monthly tuition and applicable fees will be automatically charged according to the first letter of the account holder’s last name:
Week 1: Last names A–F
Week 2: Last names G–L
Week 3: Last names M–R
Week 4: Last names S–Z
Evolve Dance Studio will provide the specific billing date assigned to each week.
- COSTUME FEES: Costume fees are due in full by December 1 of each dance season and will be automatically charged to the payment method on file.
- RECITAL AND ADDITIONAL FEES: Recital and other applicable fees will be automatically charged on their designated due dates.
- NO REFUNDS OR REIMBURSEMENTS: All tuition and fees paid before a class cancellation or withdrawal are final and nonrefundable. No refunds, reimbursements, or account credits will be issued for tuition, costume fees, recital fees, competition fees, registration fees, or other prepaid expenses.
- LATE FEES: A $5 late fee per family will be applied if payment fails or is not received by the assigned due date.
- PAYMENT FAILURE: If a valid payment method is not maintained for more than one billing cycle, the dancer’s class placement may be forfeited. All fees already paid remain nonrefundable, and the account holder remains responsible for any outstanding balance.
- TUITION STRUCTURE
* Tuition is based on a membership-style program. The total tuition for the dance season is calculated annually and divided into equal monthly payments.
* Monthly class fees are not prorated for any reason, including late enrollment, withdrawal during a billing period, missed classes, holidays, scheduled breaks, weather closures, illness, vacations, or scheduling conflicts.
* Monthly tuition remains the same regardless of the number of classes held during a particular month.
* Tuition is due in full regardless of attendance.
* No refunds, reimbursements, or credits will be provided for missed classes.
- ATTENDANCE, ILLNESS & MISSED CLASSES
* Tuition is due in full regardless of a dancer’s attendance.
* If a class is canceled by the studio or instructor, excluding weather-related closures, a make-up class may be offered.
* Make-up classes are based on available space, scheduling, and instructor availability and are not guaranteed.
* Missed classes do not qualify for prorated tuition, refunds, reimbursements, or account credits.
- CHANGING OR DROPPING CLASSES
* Thirty days’ written notice is required to withdraw from or discontinue any class.
* The account holder remains financially responsible for all tuition and fees charged or due during the required 30-day notice period.
* A withdrawal request does not take effect retroactively.
* No refunds, reimbursements, or credits will be issued for tuition or fees paid before the cancellation or withdrawal date.
* Monthly class fees will not be prorated when a dancer withdraws during a month or billing period.
* Costume fees are due by January 1 of each dance season. Once a costume fee has been paid, it will not be refunded or reimbursed if the dancer later withdraws, regardless of whether the costume has been ordered, received, or worn.
* Withdrawal from a class does not eliminate responsibility for fees incurred before the effective withdrawal date.
- CLASS OBSERVATION
* Parents and guardians may not enter classrooms during instruction unless invited by an instructor. This policy helps minimize distractions.
* Parents and guardians must remain in the designated waiting area during class time.
- SAFETY & SUPERVISION
* Evolve Dance Studio staff cannot supervise dancers outside the classroom or designated lobby area.
* On-time drop-off and pick-up are required.
* For late pick-ups, parents or guardians must park and come inside to retrieve their dancer.
* Children under two years old may not be left unattended without a parent or guardian present.
* Dancers who are not fully potty-trained must have a parent or guardian on-site at all times.
- LIABILITY WAIVER
By enrolling at Evolve Dance Studio, the dancer and parent or guardian acknowledge and agree that:
Dance activities involve inherent risks, including but not limited to sprains, strains, fractures, falls, and other injuries.
The dancer voluntarily participates in classes, rehearsals, performances, competitions, and studio events.
The dancer and parent or guardian release and hold harmless Evolve Dance Studio, its owners, instructors, employees, and staff from liability related to participation in studio activities, including claims involving active or passive negligence, to the fullest extent permitted by law.
- INJURY DISCLAIMER
Parents, guardians, and dancers knowingly assume the risks associated with participation in dance activities.
Parents and guardians are responsible for medical expenses resulting from a dancer’s participation, illness, or injury.
- MEDIA RELEASE
Permission is granted for Evolve Dance Studio to photograph or record dancers and use those photographs or recordings for studio marketing and promotional purposes, including social media, websites, advertisements, and printed materials.
No compensation will be provided for the use of these materials.
A parent or guardian who does not consent to media use must provide written notice to Evolve Dance Studio.
- PRIVACY POLICY
* Personal information collected during registration will be used for enrollment, safety, billing, payment processing, and studio communications.
* Personal information will not be sold or shared except as required by law or as necessary for payment processing and studio operations.
* Payments are securely handled through a third-party payment processor.
* Parents and guardians may request updates to their personal information at any time.
- ACKNOWLEDGMENT & PAYMENT AUTHORIZATION
By enrolling at Evolve Dance Studio, I acknowledge that I have read, understand, and agree to all policies and waivers outlined above, including but not limited to:
* Automatic monthly payment enrollment requirements
* The monthly payment schedule based on the account holder’s last name
* All tuition, costume, recital, competition, registration, and additional fees
* The January 1 costume-fee deadline
* The policy that monthly class fees are not prorated
* Late fees and the consequences of failed or missed payments
* The 30-day written withdrawal-notice requirement
* Continued financial responsibility during the 30-day notice period
* The nonrefundable and non-reimbursable nature of all payments made before cancellation or withdrawal
I understand that enrollment authorizes Evolve Dance Studio to automatically charge my selected payment method for all required tuition and fees according to the studio’s billing schedule and my assigned billing week.
I acknowledge that failure to maintain a valid payment method or comply with the studio’s payment policies may result in late fees, forfeiture of the dancer’s class placement, and continued financial responsibility for all tuition and fees incurred.
