EVOLVE DANCE STUDIO – POLICIES & WAIVER
(Please read carefully before enrollment)
PAYMENT & BILLING POLICIES
- AUTO-PAY REQUIRED: All accounts must be enrolled in automatic monthly payments via credit/debit card or checking account.
- PAYMENT DATE: Tuition and fees are automatically charged on the 1st of each month.
- COSTUME & RECITAL FEES: Costume, recital, and additional fees will be automatically charged on their designated due dates.
- NO REFUNDS: All tuition and fees are non-refundable, including tuition, costume fees, recital fees, competition fees, and all prepaid expenses.
- IMPORTANT DATE – JANUARY 1:
Costume and recital fees become FINAL and NON-REFUNDABLE after January 1 of the dance season for ANY reason, including withdrawal, injury, illness, scheduling conflicts, or dismissal.
- LATE FEES: A $5 late fee per family will be applied if payment fails or is not received on the due date.
- PAYMENT FAILURE: If a valid payment method is not on file for more than one billing cycle, your dancer’s spot may be forfeited and all fees paid remain non-refundable.
TUITION STRUCTURE
- Tuition is a membership-style program, calculated annually and divided into equal monthly payments.
- Monthly tuition remains the same regardless of the number of classes in a month, including holidays and scheduled breaks.
- NO PRORATES / NO REFUNDS for missed classes due to illness, weather closures, vacations, or school breaks.
ATTENDANCE, ILLNESS & MISSED CLASSES
- Tuition is due in full regardless of attendance.
- If a class is canceled by the studio or instructor (excluding weather-related closures), a make-up class may be offered.
- Make-up classes are based on space and instructor availability and are not guaranteed.
CHANGING OR DROPPING CLASSES
- 30-DAY WRITTEN NOTICE REQUIRED to withdraw from any class.
- All tuition and fees owed through the withdrawal date must be paid in full.
- NO REFUNDS will be issued for any payments made prior to withdrawal.
- REMINDER: Costume & recital fees are non-refundable after January 1.
CLASS OBSERVATION
- Parents/guardians may not enter classrooms during instruction to minimize distractions.
- Parents/guardians must remain in the designated waiting area during class time.
SAFETY & SUPERVISION
- Evolve Dance Studio staff cannot supervise dancers outside of the classroom or lobby.
ON-TIME DROP-OFF & PICK-UP IS REQUIRED.
- Late pick-ups require parents/guardians to park and come inside.
- Children under age 2 may not be left unattended without a parent/guardian present.
- Dancers who are not fully potty-trained must have a parent/guardian on-site at all times.
LIABILITY WAIVER
- By enrolling at Evolve Dance Studio, you acknowledge and agree:
Dance activities involve inherent risks including, but not limited to, sprains, strains, fractures, or other injuries.
You release and hold harmless Evolve Dance Studio, its owners, instructors, and staff from any liability related to participation in classes, rehearsals, performances, or events.
This includes claims of negligence, whether active or passive.
INJURY DISCLAIMER
- Parents/guardians and dancers assume all risks associated with participation and are responsible for any medical expenses incurred.
MEDIA RELEASE
- Permission is granted for Evolve Dance Studio to use photos and videos of dancers for marketing and promotional purposes, including social media and print materials.
- No compensation will be provided.
- Written notice must be submitted if you do not consent to media use.
PRIVACY POLICY
- Personal information collected during registration is used solely for enrollment, safety, billing, and studio communication.
- Information is never sold or shared, except as required by law or for payment processing.
- Payments are securely handled through a third-party processor.
- Parents/guardians may request updates to their information at any time.
ACKNOWLEDGMENT & PAYMENT AUTHORIZATION
By enrolling at Evolve Dance Studio, I acknowledge that I have read, understand, and agree to all policies and waivers outlined above, including but not limited to:
- Automatic monthly payment enrollment requirements
- All tuition, costume, recital, competition, and additional fees
- The January 1 non-refundable costume & recital fee policy
- Late fees and consequences of failed or missed payments
- The 30-day written withdrawal notice requirement
- The non-refundable nature of all payments
I understand that enrollment constitutes authorization for Evolve Dance Studio to automatically charge my selected payment method for all required tuition and fees according to the studio’s billing schedule.
I acknowledge that failure to maintain a valid payment method or failure to comply with payment policies may result in late fees, forfeiture of my dancer’s class placement, and continued financial responsibility for all fees incurred.
