I hereby grant permission for Legacy Performing Arts (LPA) to take photos and/or videos of my child and/or myself for public and studio use, including, but not limited to, advertising, marketing, education, etc.

I agree to refrain from taking personal photos and videos of other children while they are in class.

I HAVE READ, UNDERSTOOD, AND AGREE TO BE BOUND BY THE ABOVE STATEMENT.
