Liability Waiver

LIABILITY WAIVER READ AND SIGN BELOW
I understand and acknowledge that participation in any dance class or activity, dance competition,
rehearsal, or performance involves a risk of physical injury. I voluntarily assume all risks and full
responsibility for any injury or accident that may occur to me or my child while participating in any classes,
rehearsals, performances, or activities held by Be The Light Music & Dance Company.
I hereby exempt, release, and indemnify Be The Light Music & Dance Company, including its owners, staff,
volunteers, assistants, and students from any and all liability, claims, demands, or causes of action that
may arise from injury, loss, or damage to myself, my child, or our property in connection with participation
in any studio-related activities.
Furthermore, I voluntarily waive the right to hold Be The Light Music & Dance Company, its owners,
volunteers, assistants, employees, and/or students liable for any such damage, loss, or injury. I understand
that it is my responsibility to be aware of my own or my child’s physical limitations and to avoid exceeding
them.
If I am signing this waiver on behalf of a minor, I certify that I am the parent or legal guardian and have the
legal right to waive these rights on their behalf. I have read, understand, and agree to be legally bound by
the terms of this liability waiver.
