Liability and Media

I/We, the parent(s) and/or legal guardian(s) of the student registered under this account, understand that dance is a strenuous physical activity and that serious injuries may occur. I/We agree to hold Synergy Dance Academy, its entire staff, and affiliated entities harmless for any and all claims by ourselves, our child(ren), or others accompanying us, arising from activities related to dance, including but not limited to: participation in dance classes, use of or presence on the premises, or attending or traveling to and from the studio or any off-site event.
I/We want our child(ren) under this account to fully participate with the knowledge that there is a substantial risk of injury. I/We accept and assume all risks, including but not limited to the cost of treatment, long-term or lifelong effects, and any related inconveniences or expenses, regardless of the severity of injuries. I/We specifically release Kari Herman, Uri Yochelman, Synergy Dance Academy, and all Synergy Dance Academy staff members from any and all claims, however arising, including but not limited to all physical and psychological damage.
Additionally, I/We understand and agree to the Synergy Dance Academy Drop-Off and Pick-Up Policy:
Drop-Off: Dancers may arrive no more than 10 minutes before their scheduled class time.
Pick-Up: Parents or guardians must return at least 10 minutes before class ends to ensure timely pick-up.
Synergy Dance Academy is not responsible for supervising students outside of their scheduled class times.
If I/We have other children who are not enrolled, they must remain attended in the lobby at all times. Synergy maintains a small, intimate lobby space to ensure a safe and organized environment for all dancers.
Furthermore, I/We give permission for images and videos of my child/dancer, captured during classes, rehearsals, shows, competitions, conventions, meetings, mixers, and any studio-related events, to be used solely for Synergy Dance Academy promotional materials and publications, including social media. I/We waive any rights to compensation or ownership of such media.
I/We are over 18 years of age, have read and understand all Synergy Dance Academy studio policies, and agree to comply with and abide by them in full.
