# CARRINGTON DANCE COMPANY

## Student Liability Waiver, Assumption of Risk, Medical Authorization, and Release Agreement

**Please read carefully before enrolling. By registering a student for classes, camps, workshops, rehearsals, performances, competitions, or any other activities offered by Carrington Dance Company ("Studio"), the parent/legal guardian acknowledges and agrees to the following terms:**

### 1. Assumption of Risk

I understand that participation in dance, acrobatics, aerial arts, tumbling, conditioning, fitness activities, rehearsals, performances, competitions, camps, and related activities involves inherent risks, including but not limited to:

* Slips, trips, and falls
* Muscle strains and sprains
* Joint injuries
* Fractures and broken bones
* Concussions and head injuries
* Illness or exposure to communicable diseases
* Serious bodily injury, permanent disability, paralysis, or death

I voluntarily assume all risks associated with my child's participation in Studio activities, whether known or unknown, foreseeable or unforeseeable.

### 2. Health and Fitness Representation

I certify that my child is physically able to participate in dance and related activities. I understand it is my responsibility to disclose any medical conditions, allergies, injuries, physical limitations, behavioral concerns, or special needs that may affect participation.

I agree to notify the Studio promptly of any changes to my child's health status.

### 3. Release of Liability

To the fullest extent permitted by law, I, on behalf of myself, my child, my heirs, representatives, and assigns, hereby release, waive, discharge, and hold harmless:

* Carrington Dance Company
* CAPA Squared
* Studio owners
* Employees
* Independent contractors
* Instructors
* Choreographers
* Volunteers
* Guest artists
* Facility owners and landlords

from any and all claims, demands, causes of action, damages, losses, liabilities, costs, or expenses arising from or related to participation in Studio activities, except in cases of gross negligence or willful misconduct as prohibited by applicable law.

### 4. Emergency Medical Authorization

In the event of illness, injury, or emergency, I authorize Studio staff to obtain emergency medical treatment for my child if I cannot be reached immediately.

I understand that:

* The Studio does not provide medical insurance.
* I am financially responsible for all medical expenses incurred.
* The Studio makes no guarantee regarding emergency response or treatment.

### 5. Illness and Communicable Diseases

I acknowledge that participation in group activities may expose my child to viruses, bacteria, and other communicable illnesses.

I voluntarily assume all risks related to exposure to illness while participating in Studio activities.

I agree to keep my child home when they are experiencing symptoms of illness, fever, vomiting, diarrhea, or other contagious conditions.

### 6. Photography and Media Release

I grant permission for Carrington Dance Company or CAPA Squared to photograph, videotape, livestream, and record my child during classes, rehearsals, performances, competitions, camps, and Studio events.

I authorize the Studio to use such images, recordings, and likenesses for:

* Marketing and advertising
* Social media
* Website content
* Printed materials
* Promotional videos
* Studio publications

without compensation or further approval.

### 7. Property Damage and Personal Belongings

I understand the Studio is not responsible for lost, stolen, or damaged personal property, including but not limited to:

* Cell phones
* Jewelry
* Dancewear
* Costumes
* Shoes
* Bags
* Electronics
* Personal belongings

Students bring personal items at their own risk.

### 8. Student Conduct and Safety

I understand that all students are expected to:

* Follow instructor directions
* Treat staff and fellow students with respect
* Use equipment appropriately
* Follow all safety rules and procedures

The Studio reserves the right to remove any student whose behavior creates a safety concern or disrupts instruction.

### 9. Transportation

I acknowledge that I am solely responsible for transporting my child to and from Studio activities unless otherwise arranged by me.

The Studio is not responsible for supervision before or after scheduled class times. The studio is also not responsible for my student during parades or outside functions that require studio participation.

### 10. Indemnification

I agree to defend, indemnify, and hold harmless the Studio and its representatives from any claims, damages, liabilities, or expenses arising from my child's actions or participation in Studio activities.

### 11. Acknowledgment of Policies

I acknowledge that I have reviewed and agree to abide by all Studio policies, including but not limited to:

* Registration policies
* Tuition and payment policies
* Attendance policies
* Costume and recital policies
* Camp policies
* Competition team policies (if applicable)

### 12. Agreement and Electronic Acceptance

By registering my child for any class, camp, workshop, rehearsal, performance, competition, or Studio event, I acknowledge that:

* I have read this Agreement in its entirety.
* I understand its contents.
* I understand I am giving up certain legal rights.
* I voluntarily agree to all terms and conditions.
* My electronic acceptance shall have the same force and effect as a handwritten signature.

By registering your student into this portal you have agreed to the above liability and release.
