Parent Release Form for Photography and Videography
I, the undersigned, give permission to Elevate Movement Studio to use video footage and/or photographs of my child/ward. This usage may include (but is not limited to) publicly displaying, distributing, or publishing photographs and/or videos of my child for use in materials that include, but are not limited to:
Printed materials (e.g., brochures and newsletters)
Online and offline advertising and promotion
Videos and digital images for use on Social Media.
By signing this form, I acknowledge that I am giving unrestricted permission for my child’s image to be used in print, video, and digital media. I agree that these images may be used by Elevate Movement Studio for a variety of purposes and that these images may be used without further notification. I understand that any identifying information, including surname and location, will not be used in conjunction with any video or digital images.

Yo, el/la abajo firmante, autorizo a Elevate Movement Studio a utilizar grabaciones de video y/o fotografías de mi hijo(a)/representado(a). Esta autorización incluye, pero no se limita a, la exhibición pública, distribución o publicación de dichas imágenes con fines promocionales, educativos y de comunicación.
La utilización de estas imágenes podrá realizarse en los siguientes formatos:
Materiales impresos (por ejemplo, folletos, carteles y boletines informativos)
Publicidad y promoción en medios digitales o físicos
Publicaciones en redes sociales y plataformas digitales
Al aceptar este formulario, reconozco que otorgo permiso sin restricciones para el uso de la imagen de mi hijo(a) en medios impresos, audiovisuales y digitales.
Autorizo a Elevate Movement Studio a utilizar dichas imágenes para diversos fines, sin requerir notificación adicional.
Asimismo, entiendo que no se utilizará información identificativa (como apellidos o ubicación específica) junto con las imágenes o videos utilizados.
