Auto Pay Agreement

For Dance, Acro & Acting for Camera Classes:

I hereby authorize Prestige Performing Arts to charge my credit card, on a recurring monthly cycle on the first of each month beginning on the date I have registered for Dance/Acro/& or Acting For Camera classes. I understand that the autopay process will show installments and other charges to be electronically debited to my portal account on the 25th of each month. If I do not choose another form of payment (Cash, Check or Zelle) my credit card will be charged on the 1st of the month. The last two installments (May & June tuition) will be due May 1st. I understand that a 3.05% processing fee plus ¢.30 cents will be added to all credit card transactions. I understand that Prestige Performing Arts will notify me of any costume balances or additional charges as it is debited to my account during the year and will also be subject to credit card charges if not paid for by the due dates indicated. This authorization is to remain in effect until Prestige Performing Arts has received written notification of its termination.

I have read, understand, and agree to comply with the policies as stated on Prestige Performing Arts' portal. I understand that if my child wishes to drop a Dance/Acro/& or Acting for Camera class, a written notification must be made to Prestige Performing Arts via email prior to the 1st of the month. If an email is not received, I understand that I am still responsible for that month’s tuition, as a place in class is still held for my child and that I will be charged a full month’s tuition regardless of which day of the current month I withdraw my child and give Prestige Performing Arts permission to charge that full month to my account. I also understand that registration, tuition, costumes and other payments are nonrefundable. When I choose to order a costume I am agreeing to pay for the costume in full, (both the deposit and the balance) regardless of recital participation. All costumes will be distributed when all account balances are paid.

For Theater/Musical Productions:

I understand that half of tuition is due upon registration to secure a child's spot. I understand that the remaining balance is due on the first day of class. I understand that if payments are not made by its due date, a credit card on file will be charged. A 3.05% processing fee plus ¢.30 cents will be added to all credit card transactions. I understand that all payments are non-refundable after the first day of classes begin.
