Liability Agreement

By registering, I acknowledge that I have read, understand, and agree to the following:
*I acknowledge that participation in dance and related physical activities involves inherent risks, including the risk of accident, injury, or illness. I voluntarily choose to have my child participate and assume all risks associated with participation.
*I understand that while NJSDA strives to provide a safe learning environment and appropriate supervision, injuries may occur despite reasonable precautions.
To the fullest extent permitted by law, I release and hold harmless the North Jersey School of Dance Arts (NJSDA), its owners, employees, faculty, staff, volunteers, and representatives from any and all claims, demands, causes of action, liabilities, damages, or expenses arising out of or related to my child's participation in NJSDA activities, except in cases of gross negligence or willful misconduct.
*I further understand that NJSDA is not responsible for the loss, theft, or damage of any personal property brought onto the premises or to NJSDA-sponsored events.
