I hereby acknowledge, Relevéte Dance Academy LLC. or any staff, will not be held responsible for any injuries occurring in any class or other Relevéte Dance Academy LLC. event. I clearly understand that class will involve dancing, exercise and vigorous activity. I know my/my child's medical condition and hereby state that the participant is physically fit to participate in said classes. I also, understand that students and/or parents that are disrespectful to other students, instructors and/or other Relevéte Dance Academy staff may be asked to leave the studio. Relevéte Dance Academy reserves the right to refuse service.

I understand my student may be removed from classes, performances and/or competitions if account balance is not up to date.

I understand that my student must be in class the week of any performance or competition in order to participate.

I understand that Relevéte Dance Academy and staff can remove my child from competing if student is not ready due to attendance.

I understand for Competition Teams auditions are required.
