**The 716 Dance Labs WAIVER FORM & RELEASE** Please read and digitally sign Waiver to participate
I do hereby acknowledge that I/my child intends to participate with The 716 Dance Lab Programs. I also acknowledge that I/my child will be doing so of my/his/her own free will. I understand that the teachers/advisors of The 716 Dance Labs program are trained instructors. I further acknowledge that these teachers/advisors will behave prudently in the instruction of dance and acro skills in an effort to avoid accidents and/or injuries from occurring.
Fall Season Dancers -> If you choose to withdraw from the Season Early. 3 months of tuition will be due as well as costume fees. Families must enroll in Auto-Pay in the Dance Studio Portal upon enrollment. Auto-Pay will process 7 days after the due date for balance due. Cash or check payments may be made before this date at the front desk to avoid card processing fees. NSF checks or failed Auto-Pay will incur a $20 fee, plus the original amount owed. After two failed payments, future payments must be made in cash.
I realize that participation in athletic endeavors entails the risk of injury including permanent disability, paralysis and death, which may be caused by own actions, or inactions, those of others participating in the event, the conditions in which the event takes place, or the negligence of the "releases" named below; and that there may be other risks either know to me or not readily foreseeable at the time to the participants. I understand the risk of the spread of possible illness inside The 716 Dance Lab, I will not hold The 716 Dance Lab or any of their staff responsible for such outcome.
I accept the risks regardless of the nature of the injury and/or the athletic endeavor in which I/my child will participate. I acknowledge that the term athletic endeavors are used herein includes any dance, drills, jumps, gymnastics and studionastics activities of any kind. I also waive and absolve The 716 Dance Lab, staff, advisors, sponsors, parent volunteers, or other volunteers, the staff and/or volunteers of any of the venues in which we practice or perform, or any one or more of them or their executors, administrators, heirs, next of kin, successors or assign, of and from any and all liability and responsibility.
I understand that each participant is responsible for his or her own personal health, medical, dental, chiropractic, and accident insurance coverage. I will not hold The 716 Dance Lab, staff or volunteers responsible for the possible spread of sickness to my dancer. I, my heirs, executor, and administration, intending to be legally bound, do hereby waive, release, and forever discharge any and all rights and claims for damage which I/my child may have or may hereafter accrue to me/my child against The 716 Dance Labs dance program, the owners, the teachers, advisors, sponsors, parent volunteers, or other volunteers, the staff and/or volunteers of any of the venues in which we practice or perform, for any damages which may be sustained or suffered by me/my child in connection with my association with or participation in, or rising out of travel to and/or return from any dance related site or activity affiliated with The 716 Dance Labs dance program.
COMMUNITY CONDUCT AND COMMUNICATIONS
The 716 Dance Lab values respectful, honest communication and welcomes constructive feedback from dancers and their families. Concerns regarding classes, staff, policies, fees or studio operations should be brought directly to studio management whenever possible so that we have an opportunity to address them.Families may share truthful opinions about their experiences. However, harassment, threats, bullying, discriminatory remarks, knowingly false statements, disclosure of confidential information, or online conduct that targets or compromises the privacy or safety of dancers, families or staff will not be tolerated. Conduct that substantially disrupts studio operations or creates an unsafe or hostile environment may result in a meeting with studio management and, when appropriate, suspension or termination of enrollment. Whenever reasonably possible, the family will first be notified of the specific conduct at issue and given an opportunity to address it. No family will be disciplined solely for expressing an honest opinion or submitting a lawful review.
PUBLICITY WAIVER AND RELEASE
I/ my child, will be participating as a member of The The 716 Dance Labs dance program. I agree to allow any photos, videos, and/or audio recording taken of me/my child at any event or class to be used exclusively by The 716 Dance Lab for promotional purposes. These mediums may include our website, our Facebook page, our Twitter account, our Instagram account and our YouTube account, as well as newspaper articles and press releases.
CC Fees- *There is a 3% surcharge for CC and 3.5% for ACH Checking
There is always on option for Cash or Check at the front desk to avoid fees.
I understand that if my dancer comes unprepared with a ponytail holder a $3 fee will be applied to my account.
