I hereby grant Siddhi Dance Academy and Siddhi Creative permission to photograph, film, record, and otherwise capture the likeness, image, voice, and performance of the student during classes, rehearsals, workshops, performances, and events.

I authorize the use of such photographs, videos, or recordings for educational, archival, promotional, marketing, grant-related, and publicity purposes, including but not limited to websites, social media, print materials, press coverage, and promotional videos.

I understand that:
• No compensation will be provided for the use of these materials
• All media produced shall remain the property of Siddhi Creative
• Names may or may not be used in association with images
• I waive any right to inspect or approve the finished product or its use.
