Photo/Video/Printed Name Release Waiver

By signing this form, I grant Westland Dance Co. Ltd., including its employees and contractors, the irrevocable and unrestricted right to use and reproduce photographs, video recordings, filmed adjudication footage, and/or my printed name (or that of my child or family member) for purposes including, but not limited to, publication, promotion, illustration, advertising, merchandise, or trade, in any medium now known or hereafter developed.
I understand that these materials may be used without further notification and that I will not receive any compensation for their use. I hereby release and hold harmless Westland Dance Co. Ltd. and its legal representatives from any and all claims, demands, and liabilities related to the use of these materials.
