Liability Waiver

I acknowledge that by signing this liability waiver I am agreeing to the following terms and conditions as required by Expressions Dance Academy, dba Expressions Dance and Performing Arts Center, dba Expressions Academy of Dance, dba Human Art Alliance, in conjunction with utilizing its facilities, equipment, and participating in its class programs.
Participation in activities at the facility involves physical activities using height and motion, which may result in bodily injury, or even death. By signing below, I assume all risk and consent to participate, or for my child to participate, in said activities. Having such knowledge, I agree that I will hold harmless Expressions Dance Academy, its affiliates, its officers, its shareholders, its employees, and its volunteers for any and all damages resulting from all injuries or loss suffered or sustained while on its premises and under its instruction. I, participant and/or parent/guardian, will assume responsibility for any and all medical expenses and damages which may result from my or my child's participation.
I have thoroughly read this acknowledgement of risk and waiver of liability and understand it completely.
