Kay’s Dance Movement Policies & Liability Waiver

Tuition & Billing Policies
• Tuition is charged monthly and runs from the 1st of each month.
• All families must keep a debit or credit card on file and are automatically enrolled in auto-pay.
• The card on file will be charged for monthly tuition, costumes, and any miscellaneous fees (e.g., recital fees).
• To avoid credit card processing, you may pay via cash, check, or PayPal before the 1st.
• A $25 late fee will apply for tuition not received by the 10th of the month.
• A $35 bank fee applies to all returned checks. All future payments must be made via card or cash.
• The annual non-refundable Family Membership Fee is $45, charged in your anniversary month.
• Missed classes will not be refunded or credited. Classes may be made up (see Absence section).
• No refunds will be issued for early withdrawal. A credit may be placed on your account.
• All credits expire 1 year from the issue date and may take up to 30 days to process.
• Students with unpaid balances may not attend class or participate in performances/events.
• You are responsible for all legal fees incurred by Kay’s Dance Movement due to unpaid balances.
Enrollment & Withdrawal
• Students are automatically re-enrolled each month.
• Classes are a month-long commitment.
• If you wish to withdraw for the following month, complete a Drop Form by the 15th.
Verbal. Text and Email withdrawals are not accepted.
• Kay’s Dance Movement reserves the right to cancel or reschedule any class with fewer than 5 students.
• The schedule, teaching staff, and class offerings may change at any time.
Waivers, Health, and Safety
• A signed waiver must be completed in your portal before participating in any classes or events.
• Please notify us of any health conditions or special needs via the parent portal.
• Students must report all injuries (in or outside of class) to Kay’s Dance Movement staff.
• Use of slings, wraps, or braces must be accompanied by a doctor’s note.
• A doctor’s note stating “no limitations” is required to return after an injury.
• All dancers must have been examined by a physician within the past 12 months and deemed fit for dance. Any restrictions must be documented and provided in writing.
Emergency Medical Authorization
• In the event of illness or injury, I authorize Kay’s Dance Movement to seek emergency medical care as necessary.
• I understand I am financially responsible for all resulting medical costs.
• I agree to keep emergency contact and insurance information updated in the portal.
Communication Policy
• Families must have a current email on file for updates, billing, newsletters, and schedule changes.
• Important updates will be sent via email, text, and posted on our website and Facebook.
• By enrolling, you consent to receive studio communications via email, text, and phone. Standard rates may apply.
Absences, Illness, and Weather Closures
• Please log planned absences in the DSP parent portal.
• Tuition continues during absences. No credits will be issued.
• Missed classes due to illness may be made up in an age-appropriate class.
• Weather closures will be announced via text, email, Facebook, and Instagram.
• No refunds are given for weather-related cancellations; make-up classes are available.
Infectious Disease Policy (Including COVID-19)
• Despite enhanced cleaning, there is always a risk of illness transmission.
• Participation in classes assumes acceptance of this risk.
• Staff may need to physically assist students for safety, spotting, or emergency care.
Social Media & Photography Consent
• I grant permission for Kay’s Dance Movement to photograph, film, or quote my child and use these images in promotional materials, websites, and social media.
• If you wish to opt out, you must submit a written request before the start of the season and sent to info@kaysdancemovement.com
Classroom Policies
• Students must arrive on time. Late arrivals may not be allowed to participate.
• Parents are not allowed in studios. Observation is via lobby windows or TV monitors.
• Teachers may close blinds if distractions occur.
• No gum, food (outside designated areas), or street shoes in dance areas.
• Cell phones may not be used during class.
• Dress code must be followed—see Membership Guide for details.
Student Safety & Supervision
• Students in Mini classes or older must be fully potty trained and able to use restrooms independently.
• Kay’s Dance Movement is not responsible for students before or after class times.
• No students, parents, or siblings may enter dance rooms unless actively participating and with a signed waiver.
• Children may not be left unsupervised inside or outside the building.
• Siblings and friends must be supervised and follow studio rules at all times.
• Items left behind will be placed in Lost & Found and discarded or donated after 30 days.
Parent Conduct & Facility Rules
• Parents/guardians must conduct themselves respectfully toward staff and other families.
• Disruptive or inappropriate behavior may result in dismissal from the studio without refund.
• No adults are allowed on equipment or inside studios during toddler or preschool classes.
Studio Performances
• Kay’s Dance Movement offers two performance opportunities:
o Winter Showcases at community events
o Annual Dance Concert in late spring at a professional theater
• Details will be shared as soon as confirmed.
Emergency & Safety Procedures
• Emergency drills will be held during National Safety Week.
• In case of building evacuation, students will be led to the front parking lot.
• Do not remove your child from line until a staff member has confirmed dismissal.
Acknowledgment of Risk & Legal Release
• I understand dance activities carry the risk of physical injury or property loss.
• I voluntarily assume all risks associated with participation.
• I release and hold harmless Kay’s Dance Movement, its staff, agents, and affiliates from any and all claims of liability, including those arising from negligence.
• I agree to cover any legal fees or costs incurred by Kay’s Dance Movement related to enforcement of this agreement or unpaid balances.
• This waiver is governed by the laws of the State of Oklahoma. If any portion is deemed unenforceable, the remainder shall remain in effect.Electronic Signature & Certification
• I acknowledge this agreement as legally binding.
• I certify that I am the parent or legal guardian of the registered minor child(ren), or have authority to sign on their behalf.
• I confirm that all health information provided is accurate and up to date, and I agree to update it as needed through the parent portal.
• I understand that this waiver and all policies outlined herein will remain in full effect for the entire duration of my child(ren)’s enrollment at Kay’s Dance Movement, including future seasons, unless a new waiver is issued by the studio or I submit a written request to revoke my consent.
• By submitting this agreement electronically, I confirm I have read, understood, and agree to all terms stated above.
