As a participant/parent/guardian in the program of Attitude Dance Company, LLC I recognize and acknowledge there are certain risks of physical injury and I agree to assume the full risk of any injuries, damages or loss which my child/ward or I may sustain as a result of participating in and all activities connected to or associated with this program. I agree to waive or relinquish all claims from injuries, damages, or loss which I or my child/ward may have arising out of, connected with, or in any way associated with Attitude Dance Company, LLC and its staff, agents or officers. I further give permission for myself/child to be photographed and/or video taped during any class for purposes of promoting Attitude Dance Company, including the website, Facebook, Instagram and marketing materials. I have read and understand this waiver and hereby release all claims.
All Accounts MUST BE ENROLLED in Auto-pay. Your account will be charged the monthly balance on the 1st of each month. Recreational 12 Week Session: 25% of Balance- September 1st 25% of Balance- October 1st 25% of Balance- November 1st Remaining Balance- December 1st All Star Competition Season: 20% of Balance- September 1st 20% of Balance- October 1st 20% of Balance- November 1st 20% of Balance- December 1st Remaining Balance- January 1st Studio Competition Season: 12% of Balance- The first of every month September 2025-April 2026. Remaining Balance- May 1st 2026 Any accounts with failed payment attempts will incur a processing fee of $50.00.
By enrolling in classes at Attitude Dance Company, I confirm that I have read and accepted the terms of the Payment Policies set in place below. *I agree to enroll in Auto-Pay and agree to allow Attitude Dance Company to charge my information on file on the first of every month. *If my account has insufficient funds, I will incur a $50 fee. *ADC will no longer be accepting cash or check payments. *All classes must be pre-paid before a dancer is allowed to participate in class. *Credit/Debit Card payments will have a 2.75% processing fee. ACH payments will have a 0% processing fee. * NO REFUNDS * Classes will NOT be pro-rated unless the class as a whole begins later in the session. No Exceptions. * If a class is canceled due to inclement weather, classes will not have a makeup day and will not be pro-rated. If more than 3 classes are canceled due to weather we will schedule makeup lessons.
I agree to allow Attitude Dance Company to take photo and video of myself/child while in attendance at any studio class or event. I allow Attitude Dance Company to use these photos/videos on any marketing realm.
