I hereby release Stonington Danceworks, its employees, contractors and volunteers from all responsibility for any illness or injury by participants while traveling to and from or participating in the aforementioned activity.

All dance related and outside injuries must be disclosed to Miss Mara as soon as possible to ensure the proper care is taken.
