TDS THE DANZ STUDIO LIABILITY WAIVER, PHOTO RELEASE & PAYMENT AGREEMENT
By registering for classes at TDS The Danz Studio, I acknowledge and agree to the following:
LIABILITY RELEASE
I understand that participation in physical activities such as dance, acrobatics, and aerial arts involves inherent risks, including but not limited to falls, sprains, and other injuries. I knowingly and voluntarily assume all such risks.
I hereby release and hold harmless TDS The Danz Studio, TeriLee Hammond, and all instructors, assistants, volunteers, students, guests, and any associated practice or performance venues from any and all liability, claims, demands, or causes of action for injury, illness, or accident that may occur while participating in or traveling to and from classes, rehearsals, performances, or events, whether supervised or unsupervised.
PHOTO/VIDEO RELEASE
I grant permission for TDS The Danz Studio to photograph or record my child during classes, rehearsals, and performances. I understand that these images may be used for promotional purposes, including but not limited to social media, printed materials, and the studio website. I understand that no compensation will be given for the use of these images.
PAYMENT AGREEMENT
I understand that monthly tuition is due on the first lesson of each month. If payment is not received within 15 days, a $10 late fee will be automatically applied to the account. I further agree to pay any additional fees, including attorney or collection costs, that may be incurred in the event of an unpaid balance.
ACKNOWLEDGEMENT
By completing my registration, I confirm that I have read, understood, and agree to the terms outlined in this Liability Waiver, Photo Release, and Payment Agreement.
