I understand that the direction I must drive in the parking lot is from 2821 N West St to 2822 N Izabel St.

I understand that tuition is a monthly amount. If my dancer misses class, it is my responsibility to make-up that class by attending a different one. I do not get to pay a different amount if my dancer misses class.

I understand that there are no refunds of any kind, that tuition is due the first class of the month, and that there is a 20% late fee for tuition not paid by the 3rd week of the month. I also understand that once I register for class, I must provide notice if I am going to quit coming to dance. I cannot simply pick and choose which weeks/months I would like to pay for. I understand that by registering, I am taking a place in a limited-sized class that will not be given to anyone else.

I allow my dancer's picture to be on the website given that there is no last name attached to the picture. Must opt-out of this through a different waiver online.

Liability Clause: To the fullest extent permitted by law, the total liability, in the aggregate, of Jesica Barker, any dance instructor or employee, to the Client Dancer and anyone claiming by or through the Client Dancer, for any and all claims, losses, costs or damages, including attorneys’ fees or claims expenses resulting from this Registration Agreement shall not exceed the total compensation received under this Registration Agreement. It is intended that this limitation apply to any and all liability or cause of action however alleged, unless otherwise prohibited by law.

I understand that I have enrolled my child or children at DancEvolution located in Flagstaff, AZ ,also referred to herein as the “Childrens Activity Center". The Children's Activity Center has a program whereby webcams are in use and my children are under streaming video surveillance that is used for surveillance, training and management purposes, in addition to, secure accessibility from the web in accordance with the terms and conditions associated with theSpot TVwebsite (also referred to herein as “Spot TV”). By my signature below, I hereby consent to the photographing of myself and/or my minor child(ren)and the recording of my voice and or that of my child(ren)named above.I agree that I have no rights to any of the video footage or photographs for any reason at any time.I also agree that I will not screenshot, copy, reproduce, alter, modify, or create derivative works from theContent and Service. I understand that unauthorized recording, duplication, or distribution of this copyrighted work is illegal.I assume full liability of all the terms outlined in this release for any family member’s account activity associated with my child or children. Copyrighted work includes all web streaming and video/audio recordings. I understand that legal action can be takenagainstmebySpot TVorChildren’s Activity Center for such copyright infringement.I understand that the term "photograph" as used herein encompassesstillphotographs, audio,and motion picture footage.
