1. LIABILITY WAIVER & ASSUMPTION OF RISK
- I understand that participation in dance classes, rehearsals, performances, camps, intensives, and related activities involves physical activity and risk of injury. I voluntarily assume all risks associated with participation, including but not limited to falls, collisions, muscle strain, sprains, fractures, or other injuries.
- I agree that Fitch Dance Company, its owners, instructors, staff, volunteers, and facility landlords shall not be held liable for any injury, illness, loss, or damage incurred while participating in studio activities or while on studio premises.
- I understand that it is my responsibility to inform the studio of any medical conditions or limitations that may affect participation.
- By registering my child, I acknowledge and accept these risks.
2. MEDICAL AUTHORIZATION
- Medical Treatment Authorization
In the event of an emergency, I authorize Fitch Dance Company staff to obtain medical treatment for my child if I cannot be reached.
- I understand that I am financially responsible for any medical expenses incurred.
3. PHOTO & VIDEO RELEASE
- I grant permission for Fitch Dance Company to use photographs and/or videos of my child for promotional purposes including social media, website, print materials, advertisements, and studio marketing.
- I understand that I can opt out of this at anytime by changing the setting on my dancer/s' studio account. I understand that no compensation will be provided for such use.
4. TUITION & PAYMENT POLICY
- Students registering will be charged a $15.00 registration fee with an additional $5.00 per sibling. This fee is non refundable.
- Tuition is based on total seasonal enrollment (September–May), not per class attendance.
- Tuition is charged per immediate family account. A family account may include only siblings living in the same household.Cousins, extended family members, friends, or students from separate households may not be combined under one account for billing purposes.Each household must maintain its own separate account with a responsible adult listed as the primary account holder.The primary account holder is financially responsible for all tuition, fees, costumes, and related charges associated with students listed on that account.
-Tuition is divided into equal monthly installments for convenience. Families may pay the year in advance to receive a discount, however this must be paid before November 1st.
- Monthly tuition is due on the 1st of each month.
- Accounts not paid by the 10th will incur a $10.00 late fee.
- Accounts 30 days past due may result in removal from class.
- Returned payments will incur a $25.00 fee.
- Tuition is non-refundable.
5. WITHDRAWAL POLICY
- 2 weeks notice is required to withdraw from classes.
- Tuition remains due during the notice period.
- Registration fees are non-refundable.
- No refunds will be issued for missed classes.
6. ATTENDANCE & MAKE-UPS
- Regular attendance is essential for student progress and recital participation.
- No refunds or credits for missed classes.
- Make-up classes may be offered when available but are not guaranteed.
- Excessive absences may affect recital eligibility.
7. ILLNESS POLICY
- Students should not attend class if they have:
Fever within 24 hours
Vomiting/diarrhea within 24 hours
Contagious illness
Unexplained rash
- We reserve the right to send a student home if they appear ill.
8. STUDENT & PARENT CODE OF CONDUCT
- Student Expectations
Respect teachers and classmates.
No bullying or disruptive behavior.
Follow classroom rules.
- Parent Expectations
Communicate respectfully with staff.
No classroom disruptions.
No negative discussions about students, staff, or studio families inside or outside the studio.
- The studio reserves the right to dismiss any family for behavior that disrupts the studio environment. Tuition refunds will not be issued in cases of dismissal.
9. DRESS CODE POLICY
- Students must adhere to the studio dress code for safety and uniformity.
- Proper dancewear and shoes required.
- Hair secured away from face.
- No excessive jewelry.
- No street shoes on dance floors.
- Students not in proper dress may observe class instead of participating.
10. RECITAL & COSTUME POLICY
- Recital is part of the full-season commitment.
- Costume down payment fees are due by November 30th.
- Remaining costume balances are due in February.
- Costume fees are non-refundable after orders are placed.
- Students with unpaid balances may not participate in recital.
11. COMMUNICATION POLICY
- All studio communication is sent via email, text and/or parent portal.
- Parents are responsible for reading communications.
- Social media is supplemental and not guaranteed delivery.
12. INCLEMENT WEATHER POLICY
- Studio closures due to weather will be announced via email and social media.
- Tuition is not prorated for weather-related closures.
13. STUDIO PROPERTY POLICY
- Families are responsible for damage caused by their child to studio property.
- The studio is not responsible for lost or stolen items.
14. FACILITY & PREMISES LIABILITY ACKNOWLEDGEMENT
- Historic Building Acknowledgement
I understand that Fitch Dance Company operates in a historic building that may include architectural features typical of older structures, including but not limited to: Stairs, Uneven flooring, Threshold transitions, Narrow hallways, Seasonal temperature fluctuations
- I acknowledge that these features are part of the building’s structure and agree to exercise reasonable care while on the premises. I understand that participation in studio activities and presence within the building is voluntary.
I release and hold harmless Fitch Dance Company, its owners, landlords, and staff from claims arising from ordinary risks associated with the building’s age and structure.
15. PARKING LOT & EXTERIOR PREMISES LIABILITY
- I understand that the studio parking areas, sidewalks, entrances, and surrounding exterior spaces may become slippery due to rain, snow, ice, or other weather conditions, particularly during Michigan winter months.
- I acknowledge that:
I am responsible for supervising my child in parking and exterior areas.
The studio is not responsible for accidents occurring in parking lots, sidewalks, or exterior spaces.
I assume the risks associated with entering and exiting the premises in varying weather conditions.
- I understand that during winter months, conditions may change rapidly due to snow and ice accumulation. While the studio will make reasonable efforts to maintain safe entryways, I acknowledge that complete elimination of natural weather-related hazards is not possible.
- By entering the premises, I voluntarily assume the risk of slips, falls, and weather-related conditions.
16. STAIR & SUPERVISION POLICY
- Children under the age of 10 must be supervised by a parent or guardian in hallways, stairways, and lobby areas.
- Running, climbing on railings, or unsafe behavior in common areas is strictly prohibited.
- Parents accept responsibility for their child’s behavior outside of the classroom.
17. RELEASE & INDEMNIFICATION CLAUSE
- I agree to indemnify and hold harmless Fitch Dance Company and its owners/staff from any claims arising from participation in studio activities.
18. ACKNOWLEDGEMENT OF POLICIES
I have read, understand, and agree to all studio policies and waivers outlined above.
