Heart and Sole Dance works hard to ensure children are safe from injury, but if an emergency arises and you cannot be reached, your signature below will give Heart and Sole authorization to treat the student or call an ambulance until a parent/guardian can be reached.

TERMS OF AGREEMENT
 I understand that I am signing a legal binding contract with Heart & Sole Dance.
 I understand that there is a one-time $35.00 registration fee per child and $15 per sibling.
 I understand that tuition and registration fees are non-refundable.
 I understand that tuition is due on the 1st of the month. A $10 late fee will be added to your account after the 5th if tuition has not been paid.
 Acceptable payment: AUTO PAY is now required. Please set up your Auto-pay through the Parent Portal.
 If you pay 5 or more months in advance as one payment, you will receive a 10% discount.
 If a child's tuition falls two months behind, they will not be allowed to continue classes until tuition is current.
 I understand that I must give 30 days of notice if I plan to break the contract
 I understand that any photos or videos of students, current or past, may be used for advertising, promotional or company website purposes.
RECITAL PARTICIPANTS:
 Recital is June 1st, 2025
 Dress Rehearsal will be held in May and Tech Rehearsal is at 9 am the morning of the recital. Both rehearsals are mandatory and any dancer who is absent during these rehearsals may not be allowed to perform at recital.
 I understand that a $50.00 recital fee for the first child of a family and $30 per sibling. These are for all who are participating in the year-end recital. This is will run on Auto Pay April 18th, 2025.
 I understand that costumes are ordered in December of the current season and a $85 fee per costume will be charged for each class in which the student is enrolled. This payment may be made in installments, but all costume fees must be in by December 20th . Once a costume is paid for and ordered, there are no refunds for any reason.
 I understand that I will not drop/change classes after December 20th.
 I understand that flyers and newsletters are sent home with the students or emailed/texted to parents and it is my responsibility to read them or check the Heart & Sole Dance Parents group on Facebook.
 MISSED CLASS POLICY: There is no pro-rate or refund for missed lessons or any tuition. A student can make-up a class by taking another one within 4 weeks. Student must see a staff member about which class to take. CANCELED CLASS POLICY: Any class canceled for reasons such as weather, state of emergency or unforeseen circumstance cannot be refunded. Student may take a makeup class if they wish.
 It is also understood that Heart and Sole Dance will not be held responsible for the loss or damage of any personal items, clothing or jewelry during participation in lessons and/or performances or while on the premises.
Lobby Etiquette
Heart and Sole Dance's property is to be respected. If anyone destroys property, they will be held accountable for that damage. Charges will be posted to your account to cover any damages. Food and drink must be kept and eaten in the lobby area and designated student area ONLY. Any rough housing or not abiding by lobby rules will result in the dismissal of the student for that day. Parents please make sure you and your child/children understand and follow these policies.
Harmless Waiver
The undersigned Parent/Guardian or Participant hereby holds Heart and Sole Dance, all its instructors, and/or all other organizations involved with the studio or performances, harmless from any bodily injuries sustained while participating and/or while on the premises. I understand that participating in any physical activity contains risk.
