I will not at anytime hold "Rhythm & Shoes Dance Studio, LLC," or it's successors and assigns, or any other instructor employed by the studio responsible for any injuries incurred during dance instruction (in studio or virtual), while working with Rhythm & Shoes staff, or while on the Rhythm & Shoes Property.
I acknowledge if my child drops his/her class or classes after November 15th, I am responsible for costume costs for all of his/her class(es). I acknowledge if my child drops his/her class or classes after February 15th, I am responsible for costume costs for all of his/her class(es) and tuition costs through May. If a costume is ordered for your child, you must pay the balance in order to receive your child’s costume. If your account has an outstanding balance, you must pay your entire balance including May’s installment to receive your child’s costume!
I acknowledge that my child will not receive his/her costume until all tuition and the balances on my account are paid in full. I also acknowledge my child will not be allowed to perform at the End of Season Performance unless present at the last 4 weeks of classes (unless excused) and mandatory rehearsal practices (unless teacher excused).
