Liability Release Form and Assumption of Risk
All participants and students must agree to this form before participating in any classes or rehearsals at Taylor’s Dance Academy. If the participant is under 18 years old, a parent or guardian must also agree. Admission to classes or rehearsals will not be granted if this waiver is not accepted prior to participation.
I/We acknowledge that participation in dance classes and activities carries inherent risks of personal injury. Despite the precautions taken by the studio, accidents and injuries may occur. By agreeing to this release form, I/We (the participant and parent/guardian) assume all risks associated with the use of any and all spaces used by Taylor’s Dance Academy.
I/We agree to release and hold harmless Taylor’s Dance Academy, its affiliated entities, including Total Dance Alliance, and all their respective teachers, dancers, staff members, and facilities from any and all claims, demands, or causes of action, whether now or in the future. I/We will not hold Taylor’s Dance Academy liable for any personal injury, including but not limited to scrapes, bruises, cuts, sprains, fractures, broken bones, concussions, or death, or for any loss or damage to personal property that may occur on the premises before, during, or after classes.
Furthermore, I/We agree to adhere to class and facility rules and take full responsibility for our behavior, as well as for any damage we may cause to the facilities used by Taylor’s Dance Academy.
Protection of Property
I understand and agree that it is my sole responsibility to safeguard my personal property while attending or participating in any classes, rehearsals, workshops, or performances. I hereby release Taylor’s Dance Academy, its directors, employees, and dance instructors from all liability for any loss or damage to my personal property while attending or participating in classes, rehearsals, workshops, competitions, or performances. I also agree to abide by any rules, regulations, and policies set forth by Taylor’s Dance Academy.
Medical Attention
In the event of a physical injury or medical emergency, I authorize Taylor’s Dance Academy to make necessary arrangements for transportation to a medical treatment facility. All such transportation and medical treatment will be at my sole cost and expense. In an extreme emergency, or if my child is under 18 years of age, I understand that Taylor’s Dance Academy will attempt to notify the emergency contact(s) I have provided about the condition and how to reach me.
Photo Release
Taylor’s Dance Academy reserves the right to use photographs and videos taken during classes, workshops, competitions, performances, or other affiliated events for the purposes of instruction, advertising, and promoting its programs. Students, or the parents of minor students, who do not wish to comply with this policy must notify Taylor’s Dance Academy prior to participation in class.
Acknowledgement of Possible Schedule Changes
Taylor’s Dance Academy strives to accommodate everyone’s scheduling needs. However, occasionally, schedule adjustments are necessary due to high or low enrollment. Some classes may have too many participants, while others may not have enough to run. Additionally, staff employment changes outside the studio may require schedule modifications. Taylor’s Dance Academy will make every effort to avoid schedule changes and will communicate with affected students and parents to find the best solutions.
Acknowledgement of Waiver
By agreeing to this release, I acknowledge that I have fully informed myself of the contents of this waiver and hold harmless agreement by reading it before agreeing to it. I understand that this document represents my free and voluntary act, and that no oral representations, statements, or inducements have been made apart from the written terms. I affirm that I am at least 18 years of age and competent to sign this agreement. I execute this release with full understanding and intent to be legally bound by it. I confirm that there are no health-related reasons or problems that would preclude or restrict my or my child’s participation in this activity and that I will cover any medical costs arising from injuries to me or my child. I also acknowledge that Total Dance Alliance is a licensed, accredited, and insured organization. Should I observe any unsafe conduct or conditions before, during, or after the classes, I agree to report these to the directors, administrators, instructors, or staff members as soon as possible.
