Shine Bright Dance Studio (SBDS)

By signing this form, I acknowledge and accept the following:

Liability Release

I understand that participation in dance, performing arts, and gymnastics involves inherent physical risks. I voluntarily allow my child to participate in programs at Shine Bright Dance Studio (SBDS) and agree to release SBDS, its staff, instructors, and affiliates from any and all claims, liabilities, actions, damages, costs, or expenses - including legal fees - arising from my child’s participation in any SBDS program.

Studio Policies

I confirm that I have reviewed and understand all current studio policies and procedures. I agree to abide by these policies throughout the 2025 - 2026 season.

Auto Pay & Payment Policy

I understand that enrollment at Shine Bright Dance Studio (SBDS) requires participation in Auto Pay. Tuition for classes, camps, costumes/accessories, competitions, and other applicable charges will be automatically processed on the 1st of each month.
A late fee will apply to any failed or declined transactions.
If payment is not received by the last class of the month in which it is due, my child will be removed from the class roster.
No refunds will be issued once an Auto Pay transaction has been processed.
I acknowledge that processing fees apply to both credit card and ACH payments.
Monthly tuition is prorated only at the time of initial enrollment; late-month enrollments will receive a reduced tuition for that month. All other monthly tuition charges from September through May will be as stated on the tuition schedule. Students may make up missed classes by attending a class one level above or below their regularly assigned level, at their discretion

I also understand that additional one-time fees for competitions, intensives, conventions, recitals, and costumes may be added throughout the season. Payment for these fees is expected by the specified due dates.

Photo & Video Release
I authorize Shine Bright Dance Studio to use photos and/or videos of my child for promotional purposes, including but not limited to social media, websites, advertisements, and printed materials. I understand that no compensation will be provided for the use of such media.

Emergency Authorization
In the event of an emergency where I cannot be reached, I give SBDS permission to seek medical care for my child, including transportation to the nearest hospital or urgent care facility.

Code of Conduct
I understand that respectful behavior is expected from all students and families. Continued enrollment is based on adherence to SBDS’s behavioral and participation policies.

Agreement
I have read and fully understand this Waiver & Release Form and the studio’s policies. By signing below, I accept all terms and agree to abide by the procedures outlined above.

My digital signature serves as a binding agreement for the 2025-2026 season.
