Shine Bright Dance Studio (SBDS)
2026–2027 Enrollment Agreement, Studio Policies, Waiver & Release Form
By signing this agreement, I acknowledge, understand, and agree to the policies, expectations, financial responsibilities, and waiver provisions outlined below as part of my child’s enrollment at Shine Bright Dance Studio (SBDS) for the 2026–2027 season.
Liability Release
I understand that participation in dance, performing arts, gymnastics, tumbling, cheer, and other physical activities involves inherent risks, including but not limited to injury, accidents, physical strain, and other risks associated with participation.
I voluntarily allow my child to participate in programs offered by Shine Bright Dance Studio (SBDS) and assume all risks associated with participation.
I agree to release, waive, and hold harmless Shine Bright Dance Studio, its owners, instructors, employees, volunteers, affiliates, and representatives from any and all claims, liabilities, actions, damages, costs, or expenses (including reasonable legal fees) arising from or related to my child’s participation in any SBDS class, program, rehearsal, performance, competition, event, or activity.
Emergency Medical Authorization
In the event of an emergency where I cannot be reached, I authorize Shine Bright Dance Studio and its representatives to seek necessary medical attention for my child.
This may include contacting emergency services and arranging transportation to the nearest medical facility, hospital, or urgent care center if deemed necessary.
Season Registration Fee
I understand that a season registration fee is required for all students enrolling at Shine Bright Dance Studio.
The registration fee for the 2026–2027 season is $15 and is due at the time of enrollment.
This fee reserves my student’s class placement for the season and is non-refundable once processed.
Auto Pay & Payment Policy
I understand that enrollment at Shine Bright Dance Studio requires participation in Auto Pay for recurring monthly tuition payments.
Tuition for classes, camps, costumes/accessories, competitions, intensives, conventions, recitals, and other applicable charges may be automatically processed on the 1st of each month during the main season and/or on the scheduled due date communicated by SBDS.
A late fee may be applied to any failed, declined, or past-due payment.
During the main season, if tuition payment has not been received by the final scheduled class of the month in which it is due, the student may be removed from the class roster until the account balance is current.
I understand that payments made to SBDS are non-refundable once processed. This includes registration fees, tuition payments, Auto Pay transactions, competition fees, costume payments, and other season-related charges.
Billing & Payment Information
To help reduce processing costs and continue offering affordable tuition rates, SBDS encourages families to utilize ACH/bank draft payments whenever possible for Auto Pay.
Credit card payments remain available and will include the applicable processing surcharge. Families currently using debit or bank cards are encouraged to consider switching to ACH payments, as ACH provides the convenience of automatic payments while helping reduce studio processing expenses.
Payments made by cash or check are also accepted and will not include a processing surcharge when received by the communicated due date.
I understand that all payments must be made according to SBDS payment deadlines. Accounts with outstanding balances may result in removal from classes, competition teams, performances, or other studio activities until the account is current.

I understand that additional one-time fees may be added throughout the season, including but not limited to competition fees, convention fees, intensive fees, recital-related fees, costumes, accessories, and other required expenses. Payment for these fees is expected by the communicated due dates.
Tuition & Class Attendance Policies
During the main season, monthly tuition is prorated only at the time of initial enrollment. Students enrolling after the month has begun may receive an adjusted first-month tuition amount.
Monthly tuition from September through May will remain the same as listed on the tuition schedule. Tuition will not be reduced or prorated due to holidays, scheduled breaks, recital preparation, shortened schedules, weather closures, emergency cancellations, or other calendar adjustments included as part of the annual studio schedule.
Students may make up missed classes during the main season by attending a class one level above or below their regularly assigned level, when appropriate.
Competition Team Policies (If Applicable)
Students participating on SBDS Competition Teams are expected to follow all competition team requirements, expectations, and deadlines throughout the season.
By participating on a competition team, I acknowledge that competition commitments require additional time, financial responsibility, and dedication beyond regular class enrollment. I understand that additional requirements, expectations, deadlines, and policies apply to competition team participation. Costume & Apparel Policy
I understand that costumes, accessories, tights, shoes, team apparel, and other required items may be ordered throughout the season.
Once costumes, apparel, or required items have been ordered by SBDS, payments are non-refundable and families remain responsible for all associated costs.
Families are responsible for ensuring students have all required items in proper condition and ready for performances, competitions, and events by the communicated deadlines.
Additional costs may apply for replacement items, alterations, missing accessories, or items needed due to changes in size, damage, or loss.
Fundraising Requirement
A 50/50 raffle fundraiser will be implemented during the 2026–2027 season.
All SBDS students are expected to participate by selling a minimum of $40 worth of raffle tickets.
Families who choose not to participate or do not complete the required sales may be charged a $50 opt-out fee, which will be added to their account during the final month of the season.
Communication Policy
I understand that Shine Bright Dance Studio communicates important studio information through email, text messaging, DanceStudio-Pro, and other studio-supported communication platforms.
It is my responsibility to ensure that SBDS has accurate and current contact information, including parent/guardian email addresses and phone numbers listed in DanceStudio-Pro (DSP). I agree to review and update my family’s contact information in DSP as needed and notify SBDS promptly of any changes.
Families are responsible for ensuring they are properly signed up to receive studio communications through DSP, email, and text messaging. SBDS is not responsible for missed information due to incorrect contact information, outdated account information, unsubscribed email accounts, blocked messages, or failure to regularly review studio communications.
Weather & Emergency Closures
I understand that SBDS prioritizes the safety of students, families, and staff when making decisions regarding weather-related closures, emergency cancellations, or schedule adjustments.
If classes, rehearsals, or events are canceled or modified due to weather or other unforeseen circumstances, families will be notified through email and text communication.
Tuition will not be reduced or prorated due to weather closures, emergency cancellations, or schedule adjustments. Students may make up missed classes during the main season by attending a class one level above or below their regularly assigned level, when appropriate.
Photo & Video Release
I authorize Shine Bright Dance Studio to photograph and/or record my child during classes, rehearsals, performances, competitions, events, and other studio activities.
I grant SBDS permission to use these photos and videos for promotional purposes, including but not limited to social media, websites, advertisements, newsletters, and printed materials.
I understand that no compensation will be provided for the use of these images or recordings.
Code of Conduct & Participation Expectations
I understand that respectful, positive, and appropriate behavior is expected from all students and families participating in SBDS programs.
Students and families are expected to follow all studio policies, demonstrate respect toward instructors, staff, teammates, and other families, and maintain a positive environment.
Continued enrollment and participation in SBDS classes, teams, performances, competitions, and events is dependent upon adherence to studio expectations and policies.
Studio Policies Agreement
I confirm that I have reviewed and understand all current Shine Bright Dance Studio policies and procedures for the 2026–2027 season.
I acknowledge that I have reviewed the SBDS tuition information, class policies, competition team expectations (if applicable), and other enrollment information provided by the studio.
I agree to follow all policies, procedures, deadlines, payment requirements, and participation expectations throughout the season.
My digital signature serves as a binding agreement for the 2026-2027 season and confirms my acceptance of this Enrollment Agreement, Studio Policies, Waiver & Release Form, and all SBDS policies outlined above.
