With the above in mind, I hereby release Inspire Dance Center’s staff to render temporary first aid to my child in the event of an injury or illness, and if deemed necessary by Inspire Dance Center to call a doctor and seek medical help, including transportation by an Inspire Dance Center staff member and or its representative, whether paid or volunteer, to any health care facility or hospital, or the calling of an ambulance for said child should staff find it to be necessary.

We, the staff of Inspire Dance Center recognize our obligation to make our students and their parents aware of the risks and hazards associated with the sport of Dance and/or Acrobatics. Students may suffer injuries, possibly minor, serious, or catastrophic in nature. Dance and/or Acrobatics can be dangerous and can lead to injury.

Parents should make their child aware of the possibility of injury and encourage their children to follow all the safety rules and teacher instructions.

Inspire Dance Center, its teachers and other staff members, will not accept responsibility for injuries sustained by any student during the course of dance and/or acrobatic instruction, or open workout, or in the course of competition.

With the above in mind, and being fully aware of the risks and possibility of injury involved, I consent to have my child participate in the programs offered by Inspire dance Center. I, my executors or other representatives, waive and release all rights and claims for damages that I or my child may have against Inspire Dance Center and/or its representatives whether paid or volunteer.

I also affirm that I now have and will continue to provide proper hospitalization, health and accident insurance coverage, which I consider adequate for both my child’s protection and my own protection.

I also understand that it is the parents’ responsibility to warn the child about the dangers of dance and/or acrobatics and injury. The parent should warn the child according to what parent feels is appropriate. Inspire Dance Center will only warn the child through “Safety Messages” and our teaching style and progressions.

*Tuition is DUE the first week of each month and no later than the 10th of each month. Payment of tuition secures enrollment for your child in class. If tuition is paid after the 10th a $10 late fee will automatically be added to
your account.

* A $25 return check fee will be added on ALL insufficient fund checks.

*If your child is absent due to any reason: illness, vacation, or other activities, payment is STILL due by the 10th to continue to hold the spot in class for your child. There are NO refunds for missed classes. If classes are cancelled by Inspire Dance Center due to weather we will offer make-up classes in the Spring.

*Tuition remains the same each month regardless of whether it is a 5-week month or shortened month due to holidays. Tuition is NOT Pro-rated, refunded or credited for missed classes or holidays. Any student who misses a class may make-up classes in other classes that are comparable for their age/level.

*All accounts must be paid in full by May 10th, 2025 for dancers to be able to participate in the recital.

*Recital costumes are to be paid in full by January 9th, 2025. If not paid in full, your child’s costume will NOT be ordered. If the dancer withdraws after January 9th, 2025, the parents/guardian is still responsible for full payment of costumes. NO REFUNDS WILL BE ISSUED FOR COSTUMES after January 9th, 2025.

*A Recital Fee of $30 per student/$50 per family is due by March 10th, 2025.

*DROP CLASS: If your child decides to withdraw from a class, a form must be completed or
notification is required. Any outstanding fees on the account must be paid in full. Failure to turn in a drop class form will result in continued fees/late fees until we are notified. If your dancer drops a class any time after January 9th, 2025, you will be charged a $75.00 (per class) drop class fee.

*I understand that once my child leaves the Inspire Dance Center premises, Inspire Dance Center is NOT responsible for my child.

*Photo Release: I give permission for photographs/videos of my child in dance class or
performances to be used in promotional material for Inspire Dance Center.

*I understand that dance/acrobatics are sports that carry the possibility of injury. Inspire Dance Center will provide a safe learning environment, but in the event of injury, I will not hold Colleen J. Chance/Inspire Dance Center LLC liable.

*I hereby give permission for Inspire Dance Center to administer over the counter medications (Advil,
Tylenol, etc.) to my son/daughter if necessary.

*Inspire Dance Center is NOT responsible for alterations needed on recital costumes. If your child’s costume needs altered or is a special order (bigger sizes), YOU will be responsible for extra costs/alterations.

By registering, you acknowledge that you have read and understand all policies, tuition procedures, and fees associated with dancing at Inspire Dance Center.
