I hereby release Accent School of Dance, Karen Rich, and all staff members of any, and all responsibility for personal injury and/or accidents which may occur while participating in any classes, workshops or programs. I understand that I am enrolling my child in a program of physical activity and have agreed that my student is in good physical condition. In the case of emergency, I grant permission to the staff of Accent School of Dance to take first aid or emergency measures as judged necessary for the care and protection of my child while under the supervision of the studio.

I herby agree to abide by the price structure. I understand the late fee policy and agree that any past due accounts will be responsible for late fees. I understand no further discount or refund will be made for missed classes.

I hereby release and hold harmless Accent School of Dance, Karen Rich, and all associated staff members from any and all liability for personal injury, accidents, or other incidents that may occur while participating in any classes, workshops, programs, rehearsals, or performances.

I understand that participation in dance involves physical activity and, by enrolling my child, I confirm that they are in good physical condition and able to safely participate in such activities.

In the event of an emergency, I authorize the staff of Accent School of Dance to administer basic first aid and/or take appropriate emergency measures, including seeking medical assistance, as deemed necessary for the health and safety of my child while under the studio’s care.
