Liability Waiver:

I hereby release Accent School of Dance, Karen Rich, and all staff members from any and all liability for personal injury, accidents, or damages that may occur while my child is participating in any classes, workshops, programs, or activities offered by the studio.

I understand that I am enrolling my child in a program involving physical activity and certify that my child is in good physical condition and able to participate safely.

In the event of an emergency, I authorize the staff of Accent School of Dance to administer basic first aid and to seek appropriate emergency medical care as deemed necessary for the health and safety of my child while under the supervision of the studio.

Payment Agreement:

I agree to abide by the studio's tuition and fee schedule, including all payment policies. I understand and accept the late fee policy and agree that any past-due balance may be subject to applicable late fees.

I understand that missed classes are not eligible for refunds or additional discounts unless otherwise stated in the studio's policies.
