I agree to enroll in automatic monthly payments. Payments will be withdrawn on the 1st of each month with a late fee of $15 if payment has not been received by the 10th. PLEASE READ, INITIAL AND SIGN THE RELEASE AGREEMENT

RELEASE & INDEMNITY AGREEMENT

THIS RELEASE AND INDEMNITY AGREEMENT is made by the undersigned adult (the Participant or Participant's guardian), to release and indemnify Texas Family D.R.E.A.M. Center, LLC, a Texas limited liability company, and all of its respective members, employees, heirs, successors, agents, contractors, volunteers, and assigns as set forth below.
Participant, on Participant's own behalf of the other members of the Participant's family, including Participant's spouse, parents, children, heirs, and assigns, (singularly and collectively referred to as the Participant hereby grants to the Institute this full release and indemnification as consideration in exchange for permitting Participant to participate in this program.
Participant is entering into this Release after having viewed or having the opportunity to view the facility; having reviewed the instructor's qualifications; having had the scope of the services and associated risks explained; and having had an opportunity to ask questions regarding the services and risks associated with this program.
Participant is voluntarily participating in the activity with full knowledge, understanding and appreciation of the risks inherent in any physical exercise/activity and expressly assumes all risks of injury and even death which could occur by reason of participant's participation.
Participant releases Texas Family D.R.E.A.M. Center, LLC from any liability and agrees not to sue the Institute with respect to any cause of action for bodily injury, property damage, or death occurring to participant as result of participating in this program.
Participant hereby assumes full responsibility for risks of bodily injury, property damage, or death to participant due to the ordinary negligence or gross negligence of the Institute and the ordinary negligence, gross negligence, or willful misconduct of any third party including others participating in the program.
Participant agrees to indemnify, defend, and hold harmless, at Participant's sole cost the Institute from any and all claims arising out of Participant's participation in the program.
All personal property brought to the Institute is brought at the sole risk of the Participant as to its theft, damage, or loss.
Participant expressly agrees that the terms of release indemnity contained herein are intended to be as broad and inclusive as is permitted by the laws of Texas. Any provision of this Release found to be invalid by the courts having jurisdiction shall be invalid only with respect to such provision or portion.

Texas Family D.R.E.A.M. Center Release & Indemnity Agreement

By signing, the participant or guardian allows the dancer to participate in programs at Texas Family D.R.E.A.M. Center.

The participant understands and accepts the risks involved with physical activities, including possible injury or death.
The participant agrees not to sue Texas Family D.R.E.A.M. Center or its staff for injuries, property damage, or death that may occur while participating.
The participant takes full responsibility for risks, even if caused by negligence from the facility or others in the program.
The participant agrees to protect and cover the studio from any claims or legal costs related to the participant’s involvement
Personal belongings are brought at the participant’s own risk (the studio is not responsible for loss, theft, or damage).

Injury Recovery Guidance
Recovering from a dance injury can take time, but with proper care dancers can safely return to performing. Key recovery strategies include active rest to allow healing while maintaining strength and mobility, physical therapy to rebuild flexibility and range of motion, and a gradual return to activity to avoid re-injury. Proper nutrition, hydration, and extra protein help support the body’s healing process, and maintaining good mental health is also important during recovery. Most dancers need several weeks to fully return to normal activity after being cleared to resume classes.

The agreement is intended to be as broad as Texas law allows, and if one part is invalid, the rest still applies.

Photo Consent
Parents/guardians may choose to allow or deny permission for the studio to take and use photos or videos of the participant for marketing or promotional purposes.

Tumbling in Routines
If a dancer tumbles in a routine:
The studio is not liable for injuries related to tumbling.
Tumbling must be approved by the choreographer and tumble director.
The dancer must stay enrolled in a tumble class during competition season.
If a skill becomes unsafe, the instructor can remove it from the routine until it is safe again.

Final Acknowledgment
The participant/guardian confirms they read, understand, and voluntarily agree to the terms by signing and dating the form.

Family DREAM Center and DREAM Team Baseball process tuition on the 1st of every month. If your card DOES NOT successfully process on the 1st, you will automatically be billed an additional $10 as a late fee. The fee will be recurring until a transaction has been processed successfully. If you need to update the card on file, please stop by the Family DREAM Center Office (1496 Heritage Pkwy, Suite 100) or call us at (817) 225-4144 and someone will be more than happy to help update the card on file. The balance must be paid by the 20th of each month in order for students to continue to participate in their activities.
Family DREAM Center and DREAM Team Baseball run tuition on a billing cycle. Tuition will be charged on the 1st of the month for every month in that specific season. We take our season fees and divide them up evenly between the number of months in the season. If you can not make the payment on the first, or the transaction is declined, you have until the 20th of month to log in to your Parent Portal account and make the payment. If we have NOT received the payment by the 21st, your student will not be able to participate in his or her activity. If an unforeseen circumstance arises, please call our office at (817) 225-4144 to let us know and we can see how we can help.

FOR DANCE: I understand that unless a class is specifically labeled "NON-RECITAL" that it is considered a recital class and I will be charged additional fees to pay for recital. The fees and fee schedule can be found in my handbook. I understand that I am agreeing to pay those fees by checking this box and they are non-refundable if I cancel the class.
If your child is enrolling in Baseball, Music, Theater, or Tumble only then these fees do not apply. If you are enrolling for a summer dance class or a camp at Dream, these fees do not apply.
