I hereby release and hold harmless In the Wings Academy of Dance, its director, teachers, employees, and staff from and against any liability or claim for any loss of property, injury, misadventure, harm, cost, or damage sustained by me and/or my child as a result of my child’s participation in classes, performances, and activities of In the Wings Academy of Dance.

I acknowledge the contagious nature of COVID-19 and voluntary assume the risk that I and/or my children may be exposed to or infected by COVID-19 by attending and participating and that such exposure of infection may result in personal injury, illness, and permanent disability.

I understand that the risk of being exposed to or infected by COVID-19 and other contagious diseases and viruses may result from the actions, omissions or negligence contractors, volunteers, and program participants and their families.

I release In the Wings Academy of Dance LLC, it’s instructors, independent contractors and all associates from liability for harm, injury or death pertaining to COVID-19 and other contagious diseases and viruses.

I understand and agree to In the Wings Academy of Dance LLC policies and procedures and agree to all rules and regulations.

I understand that my student must have this release form signed prior to attending classes.

I give my consent for images (photographs, video) of my dependent to be taken and used to document the activities of In the Wings Academy of Dance. I grant In the Wings Academy of Dance permission to use the images for educational and promotional purposes. I understand that if I do not want images of my dependent to be used, I will indicate this in writing.

I release In the Wings Academy of Dance LLC, it’s instructors, independent contractors, and all other associates from liability for harm, theft, or injury that may be suffered by me and/or members of my family traveling to or from or during participation in activities and programs sponsored by In the Wings Academy of Dance LLC.

I hereby acknowledge that I am voluntarily assuming full responsibility for all risks of physical injury arising out of active participation in a dance class or other dance related activities.

I understand it is my full responsibility to ensure my child is healthy and has no symptoms prior to participating in classes, activities and performances associated with In the Wings Academy of Dance, LLC.

I am aware tuition is due by the first of the month. A $30.00 late fee will be incurred if tuition is not received by the 10th of the month. A $40.00 charge is incurred for all returned checks. I understand the annual tuition is one set cost divided into ten equal payments between September 2025 and June 2026, regardless of the weeks/classes in the month. No refunds.
