By registering for classes and programs at Eloquent Movement Dance (“EMD”), I acknowledge that dance and physical activity involve inherent risks. I agree to follow all studio policies and understand that participation may involve physical exertion and performance activities.

I release and hold harmless Eloquent Movement Dance, its staff, instructors, and affiliates from liability for injuries or damages that may occur during participation in classes, rehearsals, performances, or studio events.

I also authorize emergency medical treatment if necessary and confirm that all medical and contact information provided is accurate.

By enrolling in Eloquent Movement Dance, I acknowledge and agree to the following:

• Participation in dance classes, rehearsals, performances, competitions, conventions, and related activities involves physical activity and an inherent risk of injury. I voluntarily assume all risks associated with participation.

• I release and hold harmless Eloquent Movement Dance, its owners, staff, instructors, assistants, and affiliates from liability related to injuries, accidents, damages, or incidents that may occur during participation.

• Tuition is due on the 1st of each month. Accounts not paid by the 3rd are considered late and are subject to a $25 late fee.

• Tuition and registration fees are non-refundable and reserve the dancer’s placement in class, rehearsals, performances, and/or the program.

• Enrollment confirms commitment to scheduled classes, rehearsals, performances, competitions, conventions, and all studio expectations throughout the season.

• Competition families are responsible for all applicable competition, costume, convention, travel, and performance fees by the assigned due dates.

• Eloquent Movement Dance reserves the right to update schedules, policies, tuition, fees, dress code requirements, performance expectations, and program requirements as needed throughout the season.

• Failure to maintain an active account in good standing may result in restricted participation in classes, performances, competitions, conventions, or access to the parent portal.

• Parents and guardians are responsible for reviewing all studio communications, BAND updates, portal announcements, schedules, and payment information regularly.

• Enrollment in Eloquent Movement Dance confirms acknowledgment and acceptance of all studio policies and procedures.

Dancers enrolled at Eloquent Movement Dance are expected to maintain a professional appearance and follow all studio dress code requirements for classes, rehearsals, performances, competitions, conventions, and studio events.

I understand and agree to the following:

• Dancers must arrive to class in proper dance attire, appropriate dance shoes, and with hair secured neatly away from the face unless otherwise instructed.

• Required class attire, shoes, costumes, warm-ups, teamwear, and appearance expectations may vary by class, age division, performance group, or competition team.

• Jewelry, gum, inappropriate clothing, or items that may interfere with safety, technique, or instruction are not permitted during class or performances.

• Competition dancers are expected to maintain performance-ready appearance standards including hair, makeup, accessories, tights, shoes, and costume preparation as instructed by Eloquent Movement Dance staff.

• Parents and dancers are responsible for ensuring all required dancewear, shoes, costumes, and appearance items are purchased and prepared by assigned deadlines.

• Dancers who arrive improperly dressed may be asked to observe class, sit out of rehearsal, or may not be permitted to participate in performances, competitions, media opportunities, or studio events.

• Eloquent Movement Dance reserves the right to update dress code and appearance requirements throughout the season as needed for performances, branding, safety, or instructional purposes.

In the event of an illness, injury, accident, or medical emergency involving the participant during participation in Eloquent Movement Dance (“EMD”) classes, rehearsals, performances, competitions, conventions, camps, travel events, or studio activities, I authorize EMD staff, representatives, instructors, or designated personnel to obtain emergency medical care if I cannot be reached immediately.

I understand and agree to the following:

• I authorize emergency medical treatment, transportation, hospitalization, first aid, and other medical services deemed necessary for the participant’s safety and well-being.

• I understand that EMD will make reasonable efforts to contact a parent/guardian or emergency contact prior to obtaining medical treatment whenever possible.

• I certify that all medical information, allergies, medications, injuries, physical limitations, emergency contacts, and health concerns provided to EMD are accurate and up to date.

• I understand that it is my responsibility to notify EMD immediately of any medical condition, injury, illness, allergy, restriction, or change in health status that may affect participation.

• I acknowledge that participation while sick, injured, or medically restricted may increase the risk of further injury or complications.

• I accept full financial responsibility for any medical expenses, emergency transportation, treatment costs, hospital fees, prescriptions, or related healthcare services incurred.

• I release and hold harmless Eloquent Movement Dance, its owner, instructors, staff, assistants, volunteers, affiliates, contractors, and facility partners from liability related to emergency medical treatment decisions made in good faith.

By enrolling in a payment arrangement with Eloquent Movement Dance (“EMD”), I understand and agree to the following:

• Payment plans are offered as a courtesy and do not reduce, modify, or waive the total balance owed for tuition, competition fees, conventions, costumes, choreography, travel, team gear, workshops, or specialty programs.

• All scheduled payments must be completed on or before the assigned due dates listed in the Parent Portal or payment arrangement.

• Competitive Team payment plans may include monthly, bi-weekly, split-payment, installment, or approved partial-payment arrangements.

• Enrollment in a payment plan does not guarantee participation eligibility if the account becomes delinquent.

• Missed, declined, disputed, reversed, or returned payments may result in late fees, suspended Parent Portal access, removal from rehearsals, performances, competitions, conventions, travel events, specialty choreography, or temporary/permanent removal from the program until the account is brought current.

• Families enrolled in Competitive Team programs may be required to maintain Auto-Pay enrollment for approved installment arrangements.

• EMD reserves the right to deny, modify, cancel, or revoke payment arrangements due to repeated late payments, non-payment, excessive delinquency, chargebacks, or failure to comply with studio policies.

• All payments made toward tuition, registration, competition fees, convention fees, costumes, travel, deposits, choreography, workshops, team gear, and specialty programs are non-refundable and non-transferable.

• Parents/guardians remain financially responsible for the full season balance regardless of attendance, withdrawal, injury, dismissal, schedule conflicts, or voluntary removal from the program.

Eloquent Movement Dance (“EMD”) respects and values the privacy of all dancers, parents, and families. Information collected during registration, enrollment, billing, communication, competitions, events, and participation is used solely for studio operations and administrative purposes.

By enrolling with EMD, I understand and agree that:

• Personal information submitted through registration forms, waivers, billing systems, and parent communications may be used for enrollment management, scheduling, attendance tracking, payment processing, emergency contact purposes, and studio communications.

• EMD may communicate through email, phone calls, text messaging, BAND, Studio Pro notifications, social media groups, and other approved communication platforms regarding classes, schedules, performances, competitions, travel, events, billing, and studio updates.

• Payment information is processed securely through third-party payment providers. EMD does not store full payment card information directly.

• EMD does not sell, trade, or knowingly share personal information with outside parties except when necessary for competitions, conventions, performances, travel arrangements, emergency situations, legal requirements, or authorized business operations.

• Photos and videos may be shared publicly only in accordance with the signed Media Consent Release.

• Parents and dancers are responsible for maintaining the confidentiality of their Parent Portal login information and account access.

• EMD reserves the right to update policies, procedures, and privacy practices as needed.

I understand and agree that tuition is due on the 1st of each month. Any unpaid balance after the 1st is considered late and subject to a $25 late fee. Failure to maintain a current account balance may result in restricted Parent Portal access, suspension from classes, rehearsals, performances, competitions, events, or removal from the program until the account is brought current.

I understand that all tuition, registration fees, competition fees, convention fees, costume payments, travel payments, deposits, and other studio-related fees are non-refundable and non-transferable.

Enrollment reserves my dancer’s placement in class and confirms commitment to the selected program schedule, rehearsal schedule, performances, competitions, and studio activities.

I understand that competition, performance, convention, costume, travel, and specialty program fees may be billed separately throughout the season and must be paid by the assigned deadlines to maintain participation eligibility.

Returned payments, declined transactions, disputes, or chargebacks may result in additional fees and immediate account review.

Eloquent Movement Dance reserves the right to update tuition, fees, payment schedules, policies, and participation requirements as needed during the season.
