I confirm that I am 18 years of age or older and I have read, understand, and agree to all Eloquent Movement Dance policies and the liability waiver.

Regular attendance is important for dancer growth. I understand that two or more consecutive missed classes may affect participation in performances, competitions and Suspension or Removal from the Team. No tuition refunds will be provided for missed classes.

I authorize Eloquent Movement Dance to automatically charge my saved payment method on the 1st of each month for tuition and any applicable fees. I will notify EMD in writing if I wish to cancel this authorization.

I understand that participation in any dance, fitness, or movement-based activity at Eloquent Movement Dance (EMD) involves physical exertion that may result in injury. I assume full responsibility for any personal injury, loss, or damage and waive, release, and discharge EMD, its staff, and representatives from any and all liability, claims, or demands arising from participation in EMD programs or activities.

I give permission for EMD to use photographs and video recordings of my child/myself for promotional use, including but not limited to social media, flyers, website content, and advertising. I understand that no compensation will be provided for the use of such media.

In the event of a medical emergency, I authorize Eloquent Movement Dance staff to obtain medical treatment for my child/myself if I am unreachable. I understand that every effort will be made to contact me first. I accept responsibility for any medical expenses incurred.

EMD values your privacy. Personal information collected during registration is used only for administrative purposes, class communications, and payment processing. We do not sell or share your information with third parties.

I understand that tuition is due on the 1st of each month with a grace period through the 3rd. A $25 late fee is applied after. All tuition, registration, and costume fees are non-refundable. I agree to the posted rates and understand that failure to pay may result in suspension from classes or program.
