Waiver and Release from Liability
I agree to allow myself and/or my child, in consideration of being allowed to participate in any classes, instructions, programs, and/or events offered by or at Keystone Ballet Academy (hereinafter referred to as Dance Classes.
I understand and agree the nature of the activities I and/or my child will be participating in have certain health and physical risks and that I and/or my child are in the proper physical condition and capable of participation in the related activities, understanding that Keystone Ballet Academy is not in any way responsible for making such a determination.
I acknowledge and agree that it is my responsibility to maintain my own accident and health insurance coverage that provides adequate coverage for myself and/or my child participating in Keystone Ballet Academy Dance Classes, and that Keystone Ballet Academy does not provide accident or health insurance for those participating in its Dance Classes.
I assume all physical risks to myself and/or my child in connection with any Dance Classes, and further release Keystone Ballet Academy and its owners and employees from liability for negligence to the fullest extent permitted by law for any injury sustained by me and/or my child while I, he or she is enrolled in any Dance Classes, including all risks reasonable connected with such activity whether foreseen or unforeseen which may result in permanent disability or death.
I understand that any dance training program will involve the instructor touching and adjusting the student's body. This is done to help the student feel the proper placement of the head, neck, torso, arms, hands, legs, and feet. This, in turn, allows the student to execute steps and movements safely.
I will inform my child to follow all the safety rules and directions given by the teachers. I agree it is my responsibility to discuss the risks involved in taking Dance Classes at Keystone Ballet Academy so that my child is aware and understands these risks. Keystone Ballet Academy will educate the child about its safety procedures and teaching protocols.
I understand that Keystone Ballet Academy is not responsible for my child or other children under my supervision who are left unsupervised in the common areas and areas surrounding the dance studio. Additionally, I acknowledge that Keystone Ballet Academy will only be supervising my child when they are participating in scheduled Dance Classes.
I understand that Keystone Ballet Academy is not responsible for personal property that is lost, damaged, or stolen while I or my child is at or on Keystone Ballet Academy's leased area or any other area of the CANBE Innovation building where our studio is located.
I authorize and agree to the use of photographs, videos, or likenesses of myself or my child by Keystone Ballet Academy as needed for its record-keeping, advertising, and/or public relations projects, and that I have no rights to the same and will not be compensated for the same.
I am aware that a copy of the Keystone Ballet Academy Handbook is available in the office. I am aware that I can view policies of Keystone Ballet Academy on the Parent Portal and/or in the studio.
I have thoroughly reviewed the contents of this release and have read all the information. My signature is proof of my intention to execute a complete and unconditional waiver and release of all liability under the terms herein, and agreement as to all terms and conditions contained above. I am of lawful age and competent to sign this affirmation.
