Studio Policy Agreement

I have read, understand and agree to the Studio Policy Agreement
set forth by Selah School of the Arts as listed below.
By signing this form I understand and agree to:
ALL LATE FEES, PENALTIES & DISMISSALS
set forth by these policies.
Rules and Regulations
1. Tuition is due and payable to SSA on the first lesson of each month and considered late after the 10th of each month¦ AUTO PAY WILL BE RAN ON THE 10th if there was no other form of payment made prior. IF THERE IS A CHARGE BACK OR RETURN OF CHECK A $20 late fee will be charged for payments made , YOUR CHILD WILL NOT BE ALLOWED TO PARTICIPATE IN ANY CLASSES IN THE FOLLOWING MONTH UNTIL THAT MONTH IS PAID.
2. A $30 fee will be charged for returned checks, after the 2nd returned check there will be NO CHECK PAYMENTS ALLOWED. Auto Pay will be attempted to be ran every week until funds are received.
3. Accounts more than 1 month late will not be allowed to take class and are subject to dismissal from SSA.
4. Tuition remains the same each month, Aug-May, regardless of holidays, extra classes, or classes missed due to inclement weather, a students sickness, Shutdowns etc. We Guarantee at least 33 Classes Each Year.
In an event of a national crisis or epidemic which causes our doors to close in person meetings,, regular classes will continue via ONLINE, and tuition will continue to be billed each month. YOU ARE AGREEING TO CONTINUE WITH YOUR COMMITMENT EVEN THROUGH NA ONLINE PLATFORM
5. No refunds or deductions in tuition rates will be made for absences of classes regardless of the reason. Missed classes may be made up in another corresponding class. Parents should check the make-up class schedule with the Director.
6. The school is under no obligation to provide makeup classes for absences that are not the fault of the school, such as inclement weather.
7. Please deliver and pick up your child within a 10-minute tolerance before and after class. Please help me by emphasizing to your child that he/she needs to wait inside the studio until their ride arrives. I cannot be responsible for children before/after their class time due to having to begin the next class promptly. IF YOU ARE MORE THAN 10 MINS late you will be charged a late fee of $5 for every addition 5 mins late after the first 10 mins.
Attendance
8. Attending class is very important and vital for each dancer to learn, progress and be successful. Not only is it important for each child but it is also important for their class. The class is a team and to be a team you need all of your team members there!!! Regular class activity and attendance enhances technique, flexibility, strength, progress, and memory development. Because we customize each class regimen to accommodate the skill levels of each class and set choreography for each classes members and their skill level, it is CRITICAL that each student be in class.
9. ALL STUDENTS MUST BE IN THEIR PROPER DANCE UNIFORM! If they do not abide in this rule, they will not be allowed to participate. THIS INCLUDES HAIR UP IN A BUN! We have a 3 strike rule, they get 3 "Forgots" before asking to sit out in class. FIRST STRIKE- YOU WILL RECEIVE A LETTER or NOTICE BY TEXT. SECOND STRIKE- YOU WILL RECEIVE A PHONE CALL. THIRD STRIKE- THEY HAVE TO SIT OUT.
10. If there comes a point that a child has missed so many classes that it is effecting her performance and the classes ability to function as a team because the same team member is always absent, your child will be dismissed from SSA program. This assessment will be determined by Vanessa Miller.
11. There will be NO REFUNDS FOR REGISTRATION, CLASS TUITION, DANCE WEAR, RECITAL FEES OR COSTUMES ONCE PAID, THIS INCLUDES WITHDRAWAL.
Costumes
12. IT IS STUDIO POLICY THAT: COSTUMES WILL NOT BE ORDERED IF THEY AREN'T PAID FOR AT THE TIME ORDERS ARE PLACED FOR SSA. IF THEY HAVE TO BE ORDERED AFTER THE STUDIO ORDERS THEN YOU WILL INCUR THE ADDITIONAL FEES AND EXPENSE THAT IT WILL COST TO ORDER YOUR COSTUME(S) SEPARATELY!!! THIS ALSO JEOPARDIZES THE ARRIVAL OF YOUR COSTUMES FOR PICTURE DAY AND EVEN RECITAL!!! IF YOU ARE SET UP ON OUR MONTHLY PAYMENT PLAN FOR COSTUMES COSTUMES WILL NOT BE ORDERED IF YOU MISS A MONTH!
13. IT IS STUDIO POLICY TO HOLD RECITAL COSTUMES FOR ANY STUDENT WITH A PAST DUE BALANCE ON THEIR ACCOUNT UNTIL THE BALANCE IS PAID, THIS INCLUDES TUITION/ RECITAL FEES ETC.
Theater and Recital
14. ALL THEATER AND DRESS REHEARSAL PRACTICES ARE MANDATORY. IF YOU MISS YOUR THEATER REHEARSAL OR HAVE TO BE CALLED BECAUSE YOU FORGOT YOUR REHEARSAL YOU ARE SUBJECT TO BEING PULLED FROM THE RECITAL!!!
15. EVERYONE IS GIVEN THEIR THEATER SCHEDULE AND EVEN REMINDED VIA EMAIL AND WEBSITE POSTING OF SCHEDULE. THIS SCHEDULE WILL BE DIFFERENT THAN YOUR WEEKLY SCHEDULE AND TIMES, SO IT IS YOUR RESPONSIBILITY TO KNOW WHEN YOUR CLASS IS SCHEDULED FOR THAT WEEK.
16. ALL ACCOUNT BALANCES HAVE TO BE PAID IN FULL BEFORE A STUDENT IS ALLOWED TO ENTER THEATRE REHEARSALS, IT IS DUE ON THE LAST OFFICIAL CLASS OF THE YEAR. IF NOT, THEN THEY MISS THEIR THEATER REHEARSALS AND WILL NOT BE ALLOWED TO PERFORM IN THE RECITAL.
Withdrawal
17. PLEASE UNDERSTAND YOU ARE COMMITTING TO A DANCE SEASON OF AUGUST-MAY. IF YOU HAVE TO BREAK THE AGREEMENT YOU MUST GIVE US A 30 DAY NOTICE. EXAMPLE: IF YOU NOTICE US BEFORE THE 15TH THAN YOU WILL NOT BE CHARGED FOR HE FOLLOWING MONTH! IF YOU NOTICE US AFTER THE 15TH THE NEXT MONTH WILL BE BILLED. IF YOU DO NOT NOTICE US YOU WILL CONTINUE TO BE CHARGED.
WHEN YOU SIGN YOU ACKNOWLEDGE AND AGREE TO ALL THE ABOVE!
Liability Waiver
I HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN THIS ACTIVITY OR EVENT,
including by way of example and not limitation, any risks that may arise from negligence or carelessness
on the part of the persons or entities being released, from dangerous or defective equipment or property
owned, maintained, or controlled by them, or because of their possible liability without fault.
I certify that I am physically fit, have sufficiently prepared or trained for participation in the activity or
event, and have not been advised to not participate by a qualified medical professional. I certify that there
are no health-related reasons or problems which preclude my participation in this activity or event.
I acknowledge that this Accident Waiver and Release of Liability Form will be used by the event holders,
sponsors, and organizers of the activity or event in which I may participate, and that it will govern my
actions and responsibilities at said activity or event.
In consideration of my application and permitting me to participate in this event, I hereby take action for
myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:
I HEREBY WAIVE, RELEASE, AND DISCHARGE SELAH SCHOOL OF THE ARTS and all divisions
thereof of any and all liability and responsibility for injuries, sickness, pandemics, accidents, natural
disasters and/or acts of God incurred during participation in and/or instruction of camps, intensives,
private instruction, choreography or any activity I may participate.
I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability
arising from the negligence or fault of the entities or persons released, for my death, disability, personal
injury, property damage, property theft, or actions of any kind which may hereafter occur to me including
my traveling to and from this event, THE FOLLOWING ENTITIES OR PERSONS: SELAH SCHOOL
OF THE ARTS and/or their directors, officers, managers, employees, volunteers, representatives, indecent
contractors and agents, the activity or event holders, activity or event sponsors, activity or event
volunteers;
I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned
in this waiver, release and registration form from any and all liabilities or claims made as a result of
participation in this activity or event, whether caused by the negligence of release or otherwise.
The accident waiver and release of liability shall be construed broadly to provide a release and waiver to
the maximum extent permissible under applicable law.
I CERTIFY THAT I HAVE READ THIS DOCUMENT, AND I FULLY UNDERSTAND ITS CONTENT.
I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF
MY OWN FREE WILL.
PARENT / GUARDIAN WAIVER FOR MINORS (Only if student is under 18 years old)
The undersigned parent and natural guardian does hereby represent that he/she is, in fact, acting in such
capacity, has consented to his/her child or ward’s participation in the activity or event, and has agreed
individually and on behalf of the child or ward, to the terms of the accident waiver and release of liability
set forth above. The undersigned parent or guardian further agrees to save and hold harmless and
indemnify each and all of the parties referred to above from all liability, loss, cost, claim, or damage
whatsoever which may be imposed upon said parties because of any defect in or lack of such capacity to
so act and release said parties on behalf of the minor and the parents or legal guardian.
