By signing this form, you consent to the following:


· Your child will participate in dance lessons and performances and understand that it is your responsibility to ascertain whether he/she is medically fit to participate.


· Understand that should your child require medical treatment as a result of an emergency or accident, you undertake to meet all costs involved including ambulance conveyancing necessary by those in authority.


· Understand it is your responsibility as the parent or guardian to supervise your child (whether they are a student of Innis Dance Studios or not) before and after their allocated class times.


· Give permission for an IDS representative to take images of your child during class and performances and understand these images may be used for promotional purposes including all social media sites (eg IDS website, Facebook, Instagram and Snapchat).

· Give consent for the above contact details to be distributed to IDS representatives for the express purpose of communications.


· Understand that you and your child are required to follow all policies and guidelines set out by IDS.


· Understand that failure to comply with all policies and guidelines may result in termination of enrolment without notice and/or explanation.


· Understand that failure to pay all term fees and associated costs may result in termination of enrolment.


· Understand that ALL fees are non-refundable (including COSTUME PAYMENTS), and will not be refunded if you or IDS terminate your enrolment throughout the dance year.
