Acknowledgement of Risk and Liability
I hereby consent to myself/my child participating in the Unique Technique dance programs. I recognize that potentially severe injury including permanent paralysis or death can occur in any activity involving motion, including dance and related activities. I assume all related risks, both known and unknown to me of my participation, including travel to and from this activity.
I hereby confirm I am in good health and can participate and perform the activity necessary in all classes, events and workshops without endangering myself and others.

I understand that it is the express intent of Unique Technique Dance to provide for the safety and protection of myself/my child. I forever release Unique Technique Dance, its owners, employees, instructors and coaches from all damages and injuries suffered by myself/my child while under the instructor’s supervision or control of Unique Technique Dance in classes or performances of any kind.

This acknowledgement of risk and waiver of liability, having been read thoroughly and understood completely is signed voluntarily as to its consent and intent.

All tuition is due the 1st of the month. Any payments received after the 7th of the month will incur an automatic $10 late fee. If you need extra time to pay tuition please just reach out to us. Thank you!

Permission to Treat
I hereby give my permission to trained medical professionals to administer emergency medical treatment to myself/my child, should sickness or any accident occur. If any problems should occur, 911 will be called immediately.

A non-refundable $25.00 registration fee is used to hold a place in our children's programs

Tuition for the first month of class, along with the $25 registration fee is due at the time of enrollment.
Adult class sessions must be paid in full to confirm a spot in the class, no registration fee needed

Email is our main way of communication so please check regularly for our emails

Tuition is due the 1st of each month. Any payments received after the 7th of the month will automatically be subject to a $10.00 late fee. After 2 late payments auto-pay must be set up to continue with classes

Once enrolled in a class, you are committing to the full session of that particular class.

If you/your child wish to stop coming to dance classes, PLEASE email utd.office.team@gmail.com and let us know immediately 
You will be responsible for tuition until we hear from you about un-enrollment.
We have wait lists for many classes, so if you do not let us know, you could be keeping other people/children from getting into a dance class.




Ballet, tap, creative dance:
Ballet shoes, Tap shoes, tights, and dance leotard. Tutu, leg warmers, etc. OK

Tap:
Tap shoes and comfortable outfit
Musical Theater: Ballet or Jazz shoes.  Dance leotard and tights

Hip Hop:
Sneakers, (Converse/Chucks) bare feet or CLEAN/non-marking sneakers, comfortable pants and shirt

We recommend purchasing your dance attire from Creative Dancewear- 705 W Holly St in Downtown Bellingham
You can also find lots of options at second hand stores

Pole Dance:
Shorts
Tank/Crop Top/Sports Bra
Wear layers for warmth
No Jewelry on hands/arms
No Late admittance- Class closes 10 minutes after starting time

Zero tolerance for "bullying" or hate of any kind.  We are accepting of everyone here at UTD

No gum, food, candy, or drinks other than water are permitted

Please make sure to contact us BEFORE any missed classes

Cash refunds will not be given for missed classes

Studio doors open 10 minutes prior to class

Unique Technique Dance classes run with the local school district calendar, all major holidays and breaks are observed.  This does not include teacher workdays, presidents days or "bank holidays." If for any reason school is canceled (bad weather) dance classes will not be held.
