* I understand that photographs/videos in which my child/I appear may be used on television, internet, video, print or display or in class for promotional or educational purposes.

No fee adjustments are made for missed lessons. If a dancer misses one of their scheduled classes, a makeup class can be taken at any time during the term of the missed lesson. The makeup lesson must be within the same age group and organised in advance. Makeup lessons cannot be rolled into a new term.

For example: A ruby dancer misses a ballet class due to a family holiday. This dancer can take part in another Ruby class as a makeup lesson. The makeup lesson must be used in the same term as the missed lesson and must be organised in advance.

If your lesson falls on a public holiday, the same makeup lesson policy will apply.

I acknowledge and consent to allow Elite Dance to use in perpetuity:
photos and/or videos of my child/myself in publications (such as flyers/brochures), advertisements (such as newspapers or online ads), our studio’s website, or on social media sites (such as the studio’s Facebook or Instagram account). I understand that in some cases, my child and/or my name may be used. I further understand no financial compensation will be provided for the use of these photos/videos.
