WAIVER AND INDEMNITY AGREEMENT

I hereby acknowledge and agree to the following waiver and indemnity for the student(s) (hereinafter collectively referred to as "the Student" or "the Students") I am enrolling with The League Studios PTY LTD – ABN: 61 631 806 525.

In consideration of the services provided by The League Studios, including its agents, owners, officers, volunteers, participants, employees, and all other persons or entities acting in any capacity on their behalf (hereinafter collectively referred to as "THE LEAGUE STUDIOS"), I agree to release, indemnify, and discharge THE LEAGUE STUDIOS on behalf of myself, my spouse, my child/children, my parents, my heirs, personal representatives, and guardians as follows:

I acknowledge that participation in dance, body movement, aerial arts, acrobatic training, jumping, trampoline use, and any other activities or instruction provided by THE LEAGUE STUDIOS involves known and unanticipated risks that may result in physical or emotional injury, paralysis, death, or damage to the Student, to myself, to property, or to third parties. I understand that such risks cannot be entirely eliminated without compromising the nature of the activities.

These risks include, but are not limited to:

Slips, trips, and falls

Collisions with people or equipment

Injuries from improper use of equipment

Sprains, bruises, strains, lacerations, fractures, and concussions

Emotional and psychological injuries

Travel-related accidents when attending classes, training, shows, or events

I acknowledge that THE LEAGUE STUDIOS staff aim to maintain a safe environment, but they are not infallible. They may misjudge a participant's fitness, ability, or the suitability of environmental conditions. They may provide incomplete warnings or instructions, or equipment may malfunction.

I expressly agree and voluntarily accept and assume all risks associated with these activities on behalf of the Student. Participation is entirely voluntary.

I hereby voluntarily release, forever discharge, and agree to indemnify and hold harmless THE LEAGUE STUDIOS from any and all claims, demands, or causes of action related to the Student’s participation in any activities or use of THE LEAGUE STUDIOS’ equipment or facilities, including any claims alleging negligence by THE LEAGUE STUDIOS.

I certify that I have appropriate medical insurance or coverage to pay for any injury or damages the Student may cause or experience while participating. I agree to bear any related costs personally.

In the event a lawsuit is filed against THE LEAGUE STUDIOS, I agree to do so solely in Hervey Bay, Queensland, Australia. If any portion of this agreement is deemed unenforceable, the remainder shall remain in full force and effect.

By signing this document or clicking “I agree to the terms and conditions” online, I acknowledge that I may be waiving my right to pursue legal action against THE LEAGUE STUDIOS for any claims covered by this waiver.

I accept and agree that THE LEAGUE STUDIOS may refuse to enrol or permit participation in any activity at their discretion, without reason.

On behalf of the Student’s parent(s), guardian(s), or legal representative(s), I grant permission to THE LEAGUE STUDIOS and their authorised agents to photograph the Student during any activities. I understand that these images may be used in promotional materials, including websites, social media, flyers, and advertising, and that no financial compensation will be provided.

I also agree to the terms and conditions of the auto-payment system. If I wish to stop auto-payments, I must notify THE LEAGUE STUDIOS in writing at least two weeks prior.

I confirm that I am over 18 years of age, have read and understood this entire document, and agree to be legally bound by its terms.

Students should strive for perfect attendance year-round.  For students to participate in any performances they must attend regularly
Please notify the studio as soon as possible if a student will be absent from a lesson 
There are no credits given for missed lessons 
Long term absences (more than 3 weeks) without notice will automatically forfeit a student’s place in a class and Account Holder will be required to pay for all missed months
The Studio Lesson Schedule is subject to change without notice

Students are required to wear appropriate shoes and attire to all lessons; Jeans are not acceptable for any lesson
Ballet students are required to wear a leotard and tights; hair in a bun
Parents are responsible to supply all appropriate shoes & dance attire
Hair must be pulled away from the face and securely fastened
No excessive jewelry should be worn during lessons
Students should bring appropriate cover-ups to wear to and from lessons
No outside shoes allowed on the dance floors
Dance shoes must not be worn outside
The League Studios is not responsible for any loss, theft, damage to personal property

All students are given the opportunity to perform 
Parents are responsible for purchasing necessary costumes from The League Studios by the due date 
Costume payments are non refundable and non transferable
Parents are responsible for purchasing tickets to The League Studios performances in advance – The League Studios makes no guarantee that tickets will not sell out
Tickets are non refundable and no exchanges
Students must be enrolled by August to participate in November show
At the Director’s discretion a new student may be invited to perform regardless of enrolment date
Any dancer who is absent with in the 60 days prior to a show can be removed from the show at Directors discretion
Dress Rehearsals and Show may be held in place of normal class and are included in your tuition payment
No video/flash photography allowed at studio performances
Students are required to remain backstage with instructors for the length of the full show unless stated otherwise – NO PARENTS are permitted backstage unless you are a parent helper.
NO student will be released during a show – NO EXCEPTIONS
There are no regularly scheduled classes the week of Dress Rehearsal or on Performance Days – the show activities replace all classes at the studio

To enrol at our studio a parent/guardian and student must read and agree to our policies and procedures.
Most terms are 10 weeks closely following school terms.
Students will be enrolled into suitable classes according to age and experience and this is determined by a qualified dance teacher or coach.
Enrolment is for the entire year. Cancellation must be made in writing to admin@theleaguestudios.com.au.
No refunds are given for term fees paid in advance. Two weeks’ notice is required.
The studio must be contacted if a student will be absent from class for any reason.
Enrolment information must be updated at reception if it changes. This includes your email address, home address and mobile phone numbers.

Registration fee must be paid to secure your spot in classes.

Fees are payable on or before the first class of the term. If payments are not made in advance or prior to the commencement of class, your child will may not be able to participate in class until all monies due are paid in full. 

All fees are based on an annual fee divided into 4 terms. Please be aware that some terms are shorter than others, some classes are cancelled during concert/eisteddfod time. However, during production time we have many extra rehearsals which have no extra cost that more than make up for lessons missed. Our fees take into account a years worth of lessons and rehearsals, not 40 weeks of lessons.

Payments can be made by direct debit or at reception by cash in a clearly labelled and sealed envelope.Visa, Mastercard is also accepted via the parent portal (A 3% Convenience fee will be added to all online payments). 

The League Studios offers Auto Pay. Account Holder must agree to all Auto Pay terms and sign the Auto Pay Agreement Form when setting up Auto Pay.  
Overdue accounts with the exception of those with payment schedules will be charged an additional 10% if not paid within two weeks of the start of term. This fee will also be added to the whole account if scheduled payments arent met.

If parents have difficulty paying fees, please email admin@theleaguestudios.com.au or hand a note to reception to discuss financial arrangements prior to due payments.

No refund or credit is given for classes missed by the students (extreme medical conditions considered).
There are no refunds for concert fees once costumes are organised.

Classes will not be run if the temperature reaches 38 degrees to ensure the safety of the students. This will be determined by the Bureau of Meteorology.
Once the temperature reaches 38 degrees we will confirm this on the websites and on our Facebook page. You may collect students immediately. Staff will not leave students unattended on these days.
There are no refunds for cancelled classes on hot weather days. Makeup classes may be done at a similar standard or style. This needs to be organised through reception.
Please ensure that students bring their water bottles in hot weather and bring a small flannel they can wet and use to cool down.
Classes on hot weather days are adjusted to accommodate the heat. 

Dancing is an activity where injuries can occur.
Injuries must be reported to the teacher or coach by the students. This is important to be aware they have hurt themselves.
When an injury occurs at our studio we will complete an injury record for the parent. Depending on the severity of the injury we do expect the parent to take the student to a physiotherapist or appropriate medical practitioner of choice.
Students who have been injured must present a clearance report from their practitioner stating they are able to return to classes. If the injury is ongoing we must receive reports regularly so we are aware of the restrictions placed on the student.
We are not liable for personal injuries.

If your The League Studios dancer is interested in maximising their performance opportunities and experiencing an outstanding DEDICATED atmosphere – Performance Classes are for you
Performance season is 12 months; Jan-Dec
Dancers placement is done by ability and age
Dancers placement is at the discretion of the Director
Dancers member account holders are required to sign a performance team contract form.
Performance payment is based on the 12 month program and billed each term.
Performance opportunity is subject to change
Performance dancers are required to have maintained attendance in 3 classes; Ballet, Jazz & Contemporary to be eligible to join
The League Studios reserves the right to remove parents and dancers from the performance class based on behaviour, participation and poor conduct
Students may not compete as a soloist / troupe member for any other dance school while enrolled at The League Studios

We appreciate sometimes parents need to talk to teachers & coaches about their children but this may not be before, during or after class time. Please make a time outside class time when the teachers & coaches are free.
If your query about your child is lengthy or of a private nature please arrange to make an appointment with studio owners & your teacher/coach. Requests can be made in writing to reception or by emailing admin@theleaguestudios.com.au or phone the studio. Please advise if you need a specific teacher/coach at the meeting.
If you wish to lodge a complaint about another member of the studio please email or write to admin@theleaguestudios.com.au
All complaints and queries will be dealt with promptly, seriously and with sensitivity and remain confidential.

TLS Student Behaviour Policy
All students are expected to show consideration and respect to all teachers & coaches in the school and also to each other. This will ensure a happy and pleasant environment for everyone.
All students must conduct themselves with honesty and courtesy and show consideration to all teachers and coaches, fellow students, parents and staff.
Students need to remember they are a role model for other students and show leadership by setting an example through their actions and speech. They need to embrace the dance family rules.
All students need to take responsibility for adhering to the rules of our studios. To bring up concerns with the appropriate person and be willing to receive direction and coaching as appropriate.
All students need to represent a high level of integrity and avoid gossip, criticism or disparaging comments about fellow students, teachers & coaches or others involved in our studios. Have the willingness to face issues and concerns with maturity and dignity.
Be committed to giving 100% at all times.
Parents will be notified if student behaviour becomes unacceptable.

Studio Rules
• All members must clean up after themselves.
• Change areas and amenities are to be kept clean and tidy.
• Bags are to be stored out of walk ways.
• Food is only to be consumed in the kitchen and student waiting area.
• Please be aware that only recyclable items are to go in the bins provided.
• If any forks, knifes, bowls or plates are used please wash them up after using.
• There is to be no running or acrobatics in waiting areas.
• No parents allowed past reception.
• There is to be no gum chewing at the studio at any time.
• No food or drink (excluding water) is allowed into the studios. This includes all attending watching day.
• All mobile phones must be on silent whilst in class and kept in dance bags.
• The League Studios and it’s staff are not liable for any damage to or losses of personal property or valuables
• Lost property is found in the reception and is emptied at the end of every term.
• Please check for lost items.
• Ensure all dance clothing & shoes are labelled.
• Please do not use spray deodorants inside the change area. Please use outside or bring roll on deodorant. Some students are allergic to sprays which can cause breathing problems.
• All students must remain inside the studio until they are collected by their parents or a responsible adult from reception.
• No collecting from studios or acrobatics room. This is for the safety of all students and must be adhered to.
• Appropriate clothing must be worn over dance uniform to leave the building and no dance shoes may be worn outside.
• No member is permitted to record or take photographs at our studios without permission from the director or teacher/coach.
• Parents are not permitted to view class unless given special permission. This allows the students to concentrate on their class work.
• We provide an Open week at the end of term 1,2 and an Annual concert for parents to see their child’s progress and encourage them to attend these events. Parents are welcome to wait in reception. Parents at open days must supervise siblings who need to remain seated and all must watch quietly.
• Students are not permitted to bring friends to classes or rehearsals.
• Senior hip hop may contain explicit music.
• Photos and/or videos may be taken of your child and used for advertising and/or training purposes
• Any choreography learned by a student is the sole property of the Studio and may not be copied or used for any activity outside of the Studio’s scope.  Permission must be granted by the Studio for any student to use Studio choreography at any venue where the member is not with the Studio or acting on behalf of the Studio
• Check the website, facebook closed group The League Families, social media and email regularly for dates and information.
• Students need to practice to improve their skills especially when exams, competitions or shows are approaching.
• Private lessons are offered during school holidays.
• Consistent practice is required at home each week to achieve and maintain flexibility.
• At the directors discretion The League Studios reserves the right to refuse service to anyone who is enrolled and takes classes at another studio.
